Dear Prospective Exhibitor:
This letter is an invitation to participate as an Exhibitor at our annual continuing medical education symposium

entitled, 14 Annual Symposium on Current Concepts in Spinal Disorders to be held at the Aria Resort & Casino
Las Vegas in Las Vegas, Nevada on January 29-31, 2015. The course is being provided by Cedars-Sinai
in Los Angeles, CA.
Attendees will include orthopedic surgeons, neurosurgeons, physiatrists, pain management, fellows, residents, and
other physicians and spine professionals interested in the treatment of spinal disorders. The topics featured during
the course will include innovations in spine surgeries, biologics and stem cells, minimally invasive surgeries, motion
preservation, complications and controversies, and deformities and pain management. Attendance at our annual
symposium is expected to be around 350.
To participate as an Exhibitor at this symposium, we require completion of the attached Exhibit agreement,
payment of the applicable exhibit fee and full compliance with the ACCME Standards for Commercial Support. Each
exhibitor will receive a table and chair(s) based on exhibit level, acknowledgment at the conference and in the
course syllabus, posters and meeting scroll slides. Exhibitor tables will be located in Juniper Ballroom 2,3&4 in the
Aria Resort & Casino Las Vegas.

EXHIBIT AGREEMENT
This agreement confirms the participation of _______________________________________ as an Exhibitor at
14th Annual Symposium on Current Concepts in Spinal Disorders scheduled for January 29-31, 2015 at the
Aria Resort & Casino Las Vegas in Las Vegas, NV.
All companies permitted by Cedars-Sinai Office of CME to complement the conference must strictly adhere to the
following guidelines regarding their participation as an exhibitor:
•
When commercial exhibits are part of the program, arrangements for these shall not influence planning or
interfere with the presentation of CME activities;
•
Exhibit placement shall not be a condition of support for a CME activity;
•
No commercial promotional materials shall be displayed or distributed in the same room immediately before,
during, or immediately after an educational activity certified for credit; and,
•
Representatives of commercial supporters may attend an educational activity, but may not engage in sales
activities while in the room where the activity takes place, and are strictly prohibited from the sale of and/or
discussion of products while attending the educational activity.
Cedars-Sinai Medical Center will provide a table and chairs for use during the meeting. In accordance with ACCME
guidelines and CSMC policies, all exhibits will be located in the pre-function area outside the educational area.

on their exhibit level. A tent card with your company
name will be on each exhibit table.

Accommodations
Exhibit Tear Down
A limited number of rooms have been reserved for
meeting attendees at a special group rate ranging
from $185-$215/night plus taxes. Rooms at our
special rates are reserved until January 5, 2015.
After this date, reservations will be accepted on a
space and rate availability basis. Reservations are
taken on a first-come, first-served basis.
Exhibitors are encouraged to tear down by 12:30 pm on
Saturday. If you plan to have exhibit materials shipped
back, please refer to Aria Resort & Casino Las Vegas
Shipping Instructions. Materials left after 1:00 pm will
be discarded.
