FEES: 1st band - $175 additional bands (or combos) - $150 PLEASE REMIT PAYMENT WITH YOUR APPLICATION (Bands will not be scheduled until payment or purchase order is received) *Feel free to duplicate this form if you have multiple entries! • Each band will be limited to 35 minutes total time, including performance, set-up and tear down time and clinic time after the performance. • Please provide us with your set-up information by attaching a separate sheet. • Piano, drumset (no cymbals, no high hat), bass amp, guitar amp, chairs, music stands, risers, sound system with microphones and electrical outlets will be provided for every band. • Bands will be scheduled in the order applications AND payment is received.
