Render counselling service to patients and their families. To deal with rape, abuse, para suicide, criminal violence including HIV and STD providing crisis intervention. Conduct family counselling and deal with teenage pregnancy. Participate in pre‐discharge planning including placement of patients need in institutional care of discharge. Assist with care plan of children and other vulnerable dependent left at home. Investigate and trace relatives of the patient. Preparing family for family discharge. Assist patients and their families assess benefits and resource available to them. Accessing disability grants through welfare system. Liaise with employers, Department of Labour. To be familiar with patient medical condition and chronically ill‐babies. Deal with pregnant teenagers, their partner and families. Discharge arrangements of new‐born babies whose mothers died during or post birth. Conduct psycho –social assessment of patients. Attend to patient referral by the ward, opd and clinic. Obtain information from patients, relatives, employers and service provider. Investigate social circumstances: record keeping, compiling reports, and attending notice meetings as required by management structure.
