 Case Coordinators Needed -­‐ 2 Positions Available Contract -­‐ Part-­‐TimeO17.5 HOURS PER WEEK - PART TIME
We are looking for a self-­‐starter with initiative who enjoys working in a fast-­‐paced environment. As a Case Coordinator, you will use your superior assessment and case management skills to create and support mentoring relationships (matches) between Youth Volunteer Mentors and Junior Youth Mentees (children). Your dynamic, energetic and professional attitude will be instrumental in helping us to match the at risk children and youth that are on our long waiting list. QUALIFICATIONS: •

Bachelor’s degree in social work or equivalent education and experience Must have previous experience doing assessments/managing a caseload of at risk children and youth clients Must have a demonstrated knowledge of community services organizations in the GTA Must have experience managing youth/adult volunteers Must have the ability to communicate effectively in English (verbal and written) Must be able to work some evenings and occasional weekends Must have own Vehicle and valid Ontario driver’s license Ability to speak multiple languages is an asset Must be computer literate (MS Office, Excel, Google, data base management, social media, email, etc.) Understanding and knowledge of issues related to new-­‐Canadians and a commitment to working from an anti-­‐racist and anti-­‐oppression framework RESPONSIBILITIES: Work cooperatively with the Volunteer Recruiters to find volunteers in the areas of need Regularly update and manage the Google maps of waiting volunteers and maintain active communication with recruiters where volunteers are needed • To match volunteer and child with an understanding of each of their needs • To determine the indicators for success and set goals with both the parent/child and Youth Volunteer at the initial match visit HOME ASSESSMENTS • Conduct home assessments of waiting children to assess the home environments and the needs of the child being considered and make further inquires with professionals i.e. referral workers, teacher, doctor, etc. • During the home visit, provide parents with information on community resources and supports available •

• Maintain records of home visits and gather information from referral sources to further assess the needs of the child. • To arrange and complete home interviews and assessment of potential volunteers CASELOAD MANAGEMENT: • Maintain caseload of matched volunteers by providing ongoing match support through regular phone contact and meetings and maintain up to date case notes in the CYSIS database • Determine goals for the Youth Volunteer and assist with follow through for them to achieve their goals • Monitor and keep records of volunteer involvement • Provide advocacy support, recognition, training opportunities and events to volunteers through regular contact • Maintain caseload of awaiting children through regular contact and appropriate referrals and pre-­‐match activities • Maintain regular contact with Parent Support Worker • Be part of the on call team on a rotation basis for Sexual Abuse disclosures • Follow standard evaluation procedures for each case COMPENSATION These 2 positions are 17.5 hours per week with a 1 year contract. Salary is commensurate upon qualifications and experience. The agency matches Youth volunteers aged 16 – 29, to a one-­‐on-­‐one relationship with children aged 6 to 15, who are experiencing social, emotional, behavioural or cultural adjustment difficulties. The goal of the program is to provide a child with a positive role model through a mentoring relationship with a Youth.
