Synopsis In daily practice, composites are the materials most commonly used for restorative dentistry. Recent product developments, combined with clinical research into stratification, now make it possible to utilise new evolved composites with excellent characteristics from a mechanical and an aesthetic point of view. It is however, a common complaint among colleagues that the techniques of layering seem to be rather complex and it is difficult to make the right colour choice: paradoxically, they say that the appearance on the market of sophisticated materials, designed to give ever better results in the medium or long term, only makes it even more difficult to make the correct decision. In this lecture, the author would like to discuss these topics and make suggestions which aim to give high quality and predictable results every day, both from an aesthetic and clinical point of view with undeniable advantages for ourselves and for our patients. At the moment he is Lecturer at the International University of Catalona, Barcelona, for a post graduate course in Aesthetic Dentistry, Visiting professor at the “Universitè de la Mediterranee, Marseille” in the course of Refinement of Aesthetic Reconstructive Dentistry. Dr. Devoto is particularly interested in the field of Conservative, Aesthetic Restoration and is the author and co-author of various publications in specialised international journals. Achieving aesthetic objectives in these cases represents a considerable clinical challenge both on implants and natural dentition, especially when the clinician is faced with an extreme compromise of the tooth structure involving also erosion and/or abrasion phenomena. Biographical sketch Mr. Oliver Brix: The role of the prosthodontist is very critical in selecting the most suitable prosthetic treatment. The spectrum covers single restorations as well as full rehabilitations. Furthermore Veneers and Table-tops are increasingly demanded techniques, efficiently producible with e.max. Beside the indication and material specifications, the main challenge is to imitate the natural esthetics and the shade harmony at the highest level. An individual design for our patients requires a sound analysis of facial references and intra-oral aspects at the same time. A conceptual approach and work flow is a cornerstone in today’s dentistry, to fulfil the patients demands and expectations, associated with an uncompromising science background, concerning materials and color recipes. The basic principles, originally described in the 80’s were further developed and enhanced to meet the demands of patients and clinicians today. The clinician is confronted with a variety of materials including membranes and bone substitute materials to choose from. Based on scientific evidence with long-term clinical data, a clinical decision tree is presented. Newest technologies including the use of biologic mediators are currently being investigated to further enhance the treatment outcomes, improve predictability and reduce patient’s morbidity. Synopsis Modern implantology increasingly requires a “natural” prosthetic outcome. To achieve this result, implants of various diameter are now available for the surgeon to mimic at best the emergence profile of the natural teeth to be replaced. He has published numerous scientific articles in both national and international peer-reviewed journals. He is co-author of the book “Implantologia Orale” (Ed. Martina, Bologna, Italy). Co-author of the book “Implantoprotesi il ripristino dell’omeostasi tramiti restaurazioni singole” (Ed. Martina, Bologna, Italy). Co-author of the book “Techniques of bone expansion in implant surgery” (Quintessence International). These implants with emergence profiles that are similar to those of the natural teeth to be replaced require a different treatment of peri-implant tissues when compared to traditional treatment. We present a technique [Edentulous Ridge Expansion (ERE)] that achieves this goal by treating the soft tissue with a partial-thickness flap preparation and by promoting healing of hard tissue and soft tissue by secondary intention . The ERE technique is presented “step by step” in order to highlight the biological principles that guide the bone repair of the treated area. Particular attention is paid to the healing by secondary intention, the peri-implant tissue, the advantages of this technique and the recommendations that should be prescribed to the patient. The last topic is the technique of vertical expansion [Localised Management of Sinus Floor (LMSF)] that can also be a combination of horizontal and vertical. The biological principles that guide the repair and the timing of hard and soft peri-implant are discussed. The peri-implant healing time for the two surgical techniques is evaluated on the basis of clinical and radiographic examinations. Synopsis Digital impressions and computer-aided manufacturing of restorations steadily gain in importance within restorative dentistry. Irrespective of the fact that digital impressions have been introduced to dentristry more than 30 years ago with the development of the Cerec CAD/CAM system, their application in daily clinical practice was not very frequent - until recently. Today, a high number of intraoral scanners and associated CAD/CAM procedures are available. One of the factors raising the attractiveness of computerised procedures may be an increase in precision of the restoration. One further factor may be the rise in efficiency as compared to conventional procedures. Finally, the high variety of new restorative materials which were introduced for the computerised manufacturing may be another reason. High strength ceramics, lithium-disilicate glass-ceramics or non-precious alloys are already well established as restorative materials. New hybrid materials like e.g. nano-ceramics even promise to be long-lasting, highly aesthetic and less costly. They can be used in the monolithic stage, and processed chairside in the dental practice. An increase in the application of these monolithic tooth – and/or implant – borne restorations can be observed in daily clinical practice. Still, the literature on their outcomes is very scarce. The lecture will critically evalulate the possibilities that the new technologies and materials offer to the practitioner, and will highlight where they are indicated. The recession of the buccal soft tissue margin is a frequent complication of well integrated dental implants. The appearance of metallic structure or even their transparency through the thin buccal soft tissues are common reasons for patient aesthetic complaints. Moreover, bad implant installation frequently results in excessive apical dislocation of the buccal soft tissue margin of the implant supported crown. Soft tissue plastic surgical procedures and bilaminar techniques in particular, can be successfully used in combination with a pre and postsurgical prostethic approaches to increase the volume of the interdental soft tissue, to treat buccal gingival recessions and soft tissue dehiscence around dental implants and to provide the new implant supported crown with an aesthetic transmucosal emergency profile.
