This past year these clinicians cared for a record number of patients, nearly 17,000 and delivered 70,000 patient visits. With the need for our services continuing to grow, we know we play a key role in Santa Barbara’s healthcare continuum. These descriptors are truly at the heart of all that we do at SBNC and will actually allow us to increase our capacity to serve more people. While we have a tremendous responsibility to serve those who need us in the community, we also have an obligation to assure you, who support our work, that we are being excellent stewards of each and every dollar. As you know, we are experiencing a significant increase of patients—approximately 1,000 new patients each year, and that number is expected to grow dramatically in the coming years. As a result, we are using a number of forward-thinking strategies to keep pace with this increasing demand. Let me give you a few quick examples: • We are reaching out to every potential partner in the community to support our efforts. I am so incredibly proud of the collaborations we have established with other concerned communitybased organizations and businesses. These relationships provide support for our work on a multitude of levels – helping to reduce costs, preventing redundancies in services, and guiding clients to where they can best be served. • We’ve upgraded our entire IT infrastructure in anticipation of the implementation of a stateof-the-art Electronic Healthcare Records (EHR) system that will tremendously reduce overlaps in services, greatly enhance integration with other healthcare providers, and improve health outcomes. • We expanded our Fund Development Department, which is solely focused on bringing in increased financial resources to fund our unreimbursed care to the most vulnerable. • We continue to hire superior clinicians, dentists, and staff who are devoted to the mission we serve and persist in finding new insightful ways to improve our service to patients and increase our resources. • Finally, our talented and dedicated Board of Directors is committed to a strategic planning process which will further advance Santa Barbara Neighborhood Clinics’ vision for the future. Fenzi, MD A fter an extensive search, Santa Barbara Neighborhood Clinics (SBNC) was pleased to announce the appointment of Dr. Charles Fenzi as its new Chief Medical Officer. “Dr. Fenzi’s commitment to quality, affordable and accessible healthcare, and his vast credentials, recognitions and achievements, make him ideally suited to take on this critical role,“ said Cynder Sinclair, DM, Chief Executive Officer, SBNC. “With Dr. Fenzi on board, we feel more empowered than ever to deliver quality medical care to those who need it in the greater Santa Barbara community,” added Sinclair. Dr. Fenzi most recently served as a physician at Community Health Centers of Central California. He has also worked in private practice and at high profile community health organizations in New Mexico, including Roswell Clinic Corp. and Roswell Medical Group (affiliated with Lovelace Medical Center). He served as Physician Liaison for Community Health Systems, based in Franklin, TN. Dr. Fenzi has served as Clinical Professor of Medicine at the University of New Mexico School of Medicine, Medical Director of Alianza, a provider of services to people living with HIV, and Physician Advisor, Chief of Staff, Chief of Medicine and Chairman for the Infection Control Committee at Eastern New Mexico Medical Center. Dr. Fenzi has also served as President of the New Mexico Chapter of the Academy of Family Physicians. “I am indeed honored to have been provided the opportunity to work with such a skilled and devoted group of physicians, most of whom have been providing exceptional care for the most vulnerable in our community for years. SBNC has been an important Santa Barbara organization for over forty years, built on the idea that healthcare should be available to all members of the community regardless of their ability to pay. At Page 4 times, that mission required major sacrifices from all members of the organization. Having started at the grass roots level, SBNC is becoming a major healthcare provider in partnership with Cottage Health System, Sansum Clinic and the County of Santa Barbara for the uninsured and the underinsured. With the large expansion in the number of people eligible for healthcare coverage anticipated in the very near future, SBNC is developing strategies to increase its capacity. CEO Cynder Sinclair has assembled a highly competent leadership team with a lofty vision of the future. In concert with the Board of Directors, with its vast experience in a variety of fields, the leadership team and the providers, with the assistance of their wonderful support staff, we are well positioned to meet these challenging times in the delivery of healthcare in our community,” Dr. Fenzi said. Dr. Fenzi attended the University of Arizona in Tucson for his undergraduate and postgraduate work in Biology and Psychology. He received his Doctorate of Medicine with honors from the Université Libre de Bruxelles in Brussels, Belgium. He completed his residency in family medicine at St. Joseph Hospital in Stamford, Connecticut, affiliated with New York Medical College and the University of Connecticut, where he was Chief Resident from 1980-1981. Among his many awards and accolades for service to the healthcare community, Dr. Fenzi was named Family Physician of the Year and received the President’s Award from the American Academy of Family Physicians, New Mexico Chapter, and Faculty Member of the Year for work with the residency program at Eastern New Mexico Medical Center. Dr. Fenzi also served our country in the United States Navy from 1966-1969 and was decorated for valor based on his service as Senior Patrol Officer in Viet Nam. Brain Injured and Potentially Homeless - a Patient’s Story of Hope and Recovery Anna slowly climbed the stairs at our Westside Neighborhood Clinic on a chilly November morning, hoping our doctors could help her. She struggled to answer simple questions from the receptionist about her appointment. Just days before, Anna had suffered a traumatic brain injury at the hand of her angry husband. Her husband had been sent to jail, leaving Anna to raise her three young sons, one of whom is autistic. Anna’s brain injury left her disoriented, uncoordinated, unable to remember, and suffering sensory overload from the smallest sound. Our behavioral health specialist contacted the landlord to intercede on Anna’s behalf, explaining how the brain injury incapacitated her. Over the next three weeks, our behavioral health specialist worked closely with Anna, helping her find assistance in cleaning her apartment. He spoke with Anna’s three young sons, asking them to work as a team and helping them understand that losing their apartment was not an option. After two attempts, her apartment passed inspection. Anna rejoiced in knowing her family would not become homeless. Our clinicians began a treatment plan for Anna’s brain injury using a carefully combined medical and behavioral health approach. In time, the dark cloud of confusion began to lift in Anna’s life. This patient’s story illustrates how our clinicians and staff are guided by our mission to provide high quality, affordable, comprehensive healthcare to all people regardless of their ability to pay. Not just for patients who walk through our doors, but for their entire family as well. Our caring staff vigilantly looks for the many challenges our patients face, and they take heroic steps to help create a healthier person, a healthier family and a healthier community. However, our behavioral health provider soon discovered that Anna’s landlord was threatening to evict her and her three sons because their apartment was a hazard, due to multiple health violations. Anna felt overwhelmed by the amount of clutter in her apartment, but was paralyzed at the thought of cleaning it due to the effects of her brain injury, even though eviction would mean a life of homelessness for her family. When individuals know how to care for themselves and for their families they become empowered in their own care. They realize they can actually have a role in their own health. While this may sound obvious to many of us, it is not so clear to some, especially children. Simple things like brushing teeth properly, washing hands, making good eating choices and getting regular exercise can greatly mitigate health risks. Prevention can greatly reduce the onset of conditions that create a need for more extensive care, including diabetes, heart disease, tooth decay, and even some forms of cancer. SBNC stresses prevention education at all levels of care for its patients. In fiscal year 2011, Santa Barbara Neighborhood Clinics served a total of 16,932 unduplicated patients with 70,080 total patient and health education visits (includes 38,093 unduplicated medical visits and 13,056 dental visits). Medical preventive services include health education delivered through our Health Promotion Center. The Health Promotion Center provides free, bilingual health education classes and trainings, either independently or with various partners, including Sansum Diabetes Research Institute. Class topics include asthma management, pre-diabetes and diabetes prevention and management, nutrition, healthy cooking, label reading, parenting classes, support groups, communication classes, self-esteem, sex education, smoking cessation, stress management, teen health advocacy, hispanic women health advocacy and many more. In fiscal year 2011, the Health Promotion Center delivered over 7,300 patient education visits. Medical Services Isla Vista Neighborhood Clinic 970 Embarcadero del Mar Mental Health Preventive Services Santa Barbara Neighborhood Clinics’ Neighborhood Counseling Program (NCP) offers mental health and counseling services focused on prevention and early intervention. NCP increases mental health access for patients and decreases the stigma surrounding mental health because the program is managed within the primary care medical setting, and allows clinicians and counselors to work together to treat patients. NCP is geared toward treating conditions like substance abuse, post-traumatic stress disorder, anxiety, depression, sexual trauma and others, as well as medication management. This program works closely with other mental health services in the area for referrals and patient coordination. In fiscal year 2011, the Neighborhood Counseling Program provided over 1,000 mental health patient visits that focused on prevention and early intervention. Milpas Street Santa Barbara, CA 93103 (805) 884-1998 Monday – Friday 7:45 a.m. – 6:30 p.m. Dental Preventive Services Dental education makes up the primary basis of dental preventive services for Santa Barbara Neighborhood Clinics. The dental education program is very innovative and unique for the Santa Barbara community, and it is a key factor in the long-term oral and overall health of patients. Our Eastside Family Dental Clinic (EFDC) staff and volunteers provide oral hygiene education and dental supplies to every single child and every member of his/ her family who visits the clinic. EFDC collaborates with the Assistance League of Santa Barbara to deliver dental hygiene and nutrition education and oral hygiene kits. In 2011, Santa Barbara Neighborhood Clinics distributed more than 11,000 free oral hygiene kits to children, their families and the homeless. These include preventive healthcare, emergency and primary care, mental healthcare, specialized care, and dental care. No single organization can provide all of these services to all segments of our population. In South Santa Barbara County, there are a number of organizations critical to the success of our health network, including Cottage Health System, Sansum Clinic, Santa Barbara County Public Health Department and SBNC. In addition, there are numerous, non-profit Community Based Organizations (CBOs) that provide goods and services to support these main healthcare providers. Among the CBOs • Donates on average $300,000 in kind for diagnostic testing for our unisured patients each year. that SBNC collaborates with are: Direct Relief International, the Assistance League of Santa Barbara, St. Cecilia Society, Legal Aid Foundation, Unity Shoppe, United Way, Transition House, Casa Esperanza, Cancer Center of Santa Barbara, to name a few. By partnering with these organizations, Santa Barbara Neighborhood Clinics is able to provide expanded services to the community. In 2011, Santa Barbara Neighborhood Clinics served nearly 17,000 patients. Of these patients: 95% were below the federal poverty level; 35% were children; 5% were seniors: 66% were Hispanic; nearly 1,000 were homeless • Grants SBNC programmatic and operational funding – in 2011 we received $150,000. Community Based Organizations • Collaborates with SBNC and SEE International to provide opthamological tests for our uninsured diabetic patients. • Direct Relief International provides inkind donations of pharmaceuticals, medical supplies and humanitarian aid averaging $250,000 each year. • Lutheran Medical Center of New York underwrites two fulltime dentists each year saving SBNC $250,000 . Page 9 Physicians Care Locally and Abroad Financials Snapshot of SBNC Financials Fiscal year 2011: July 1, 2010 – June 30, 2011 1% 1% 15% Neil Patrick Sullivan, MD Our doctors choose to work for SBNC because they are committed to providing quality healthcare to thousands of underserved in need. It’s not surprising then when we learn that one of our physicians has taken a trip overseas to provide care to the underserved in Cambodia. For the second year, Dr. Neil Sullivan volunteered with the Cambodian Health Professional Association of America to provide free medical and dental services to the people of Cambodia. The group set up the clinic in a small hospital in Koh Kong Province (near the Thai border). During a six day visit, physicians saw more than 6300 patients for a variety of conditions. Most of the people had never seen a physician, dentist or even a toothbrush. They cannot afford medications and often resort to traditional Khmer medicine such as coining and hot suctioning over the body’s surface. Dr. Sullivan alone saw adults and children at a pace of 15-18 patients an hour! Also, he was able to perform a few minor surgical procedures such as a large cyst removal. “It was a great experience for all the volunteers and students that participated. But, most important of all, it was unbelievably appreciated by those people we were able to touch,” said Dr. Sullivan. Page 10 Source: FY 2011 Audited Financial Statement.
