Through careful stewardship of our resources, we continue to provide 17,000 individual patients per year with quality, affordable healthcare, in dignified and respectful settings. As we navigate the difficulties and challenges of the changing landscape of healthcare, we ready ourselves for the full effect of the Affordable Care Act, while seeking every new opportunity to further our mission. We are complying with the mandate that all health providers convert to electronic health records by 2014. While this is a costly undertaking, this past year we have completed transferring our paper dental charts to electronic records and have implemented electronic records in our largest medical clinic. We expect to complete this transition at our other two medical clinics next year. This project is vital to the future growth and sustainability of SBNC as we seek every opportunity to carry out our mission. As Board Chair I am extremely proud of the level of passion and professionalism each staff member has brought to bear on these projects. The dedication and commitment of staff — both those behind the scenes and those with patient contact — enable our clinicians to provide healthcare to the most vulnerable among us. With our mission to provide quality care to thousands in need, our clinicians continue to focus on both the quality and breadth of care to our community. We look forward to the potential opportunities open to us as we pursue our own independent designation as a Federally Qualified Health Center. Sincerely, Gary Gray, CPA Board Chair Page 3 SBNC’s Diabetic Retinal Screening Program T he clinicians at Santa Barbara Neighborhood Clinics (SBNC) care for a large number of patients suffering from diabetes mellitus. In helping our patients manage their diabetes, SBNC has developed a sophisticated program designed to provide tools and education to help prevent the dreaded complications of this disease. Included among the complications of uncontrolled diabetes are kidney failure and arterial vascular disease, which can lead to stroke, heart attack and loss of limbs. One of the major complications of this disease is diabetic retinopathy, which is a leading cause of blindness in the United States. The optimum control of diabetes is obtained when the patient and clinician form a true partnership in the management of the disease process. In this arrangement the patient utilizes the clinician’s expertise to guide his/ her strategy not only for controlling his/her blood sugars, but also for preventing complications. This strategy usually requires the use of several classes of medication. It also requires the development of a system of measures that allows for the early detection of problems at a time when the damage can be minimized. The patient and the clinician meet regularly to discuss these measures, which include: 1) blood pressure and weight control, 2) blood tests like the Hemoglobin A1c, lipids and kidney tests, 3) foot exam by a foot specialist and 4) screening for eye disease. machines that provide better magnification. One such device is called the “retinal camera,” which captures an image of the retina after the pupil has been dilated. This image is then examined by an expert who provides the patient’s primary clinician with a report. If the expert reports an abnormality in the retina, the clinician will make arrangements for the patient to be seen by a specialist in ophthalmology, who in turn will take measures designed to preserve the patient’s vision. Page 4 Cancer Free and Grateful A bout two years ago Ilene, a long-time volunteer and former Santa Barbara Neighborhood Clinics employee, was volunteering in one of our clinics when she mentioned to Dr. Lawton that she was having unusual post-menopausal bleeding. At Dr. Lawton’s urging, Ilene went to her own doctor that was covered by her insurance and had an annual exam which turned out to be normal. The next time she was in the clinic volunteering she mentioned that she was still bleeding but the test had been normal. Dr. Lawton was still concerned and recommended she make an appointment with her right away. After performing an ultrasound, Dr. Lawton discovered a tumor and did a biopsy. Unfortunately the biopsy came back positive for cancer. Ilene was very nervous, but Dr. Lawton took swift action and referred her to the Cancer Center of Santa Barbara. She had surgery to remove the tumor within a few weeks. Upon follow up, the pathology of the tumor and lymph nodes taken for testing revealed that all of the cancer had been removed. Ilene had a few rounds of radiation therapy and now this episode in her life is behind her. “Dr. Lawton’s sound medical diagnosis and quick action saved my life. I am a living testament to the efficient and effective medical care that is available at the Santa Barbara Neighborhood Clinics. I have personally witnessed first-hand the excellent and critical medical services that are provided for the community by the Neighborhood Clinics. Dr. Lawton is a testament to this excellence of care, and my family will forever be grateful to her,” wrote Ilene. When individuals know how to care for themselves and for their families they become empowered in their own care. They realize they can actually have a role in their own health. While this may sound obvious to many of us, it is not so clear to some, especially children. Simple things like brushing teeth properly, washing hands, making good eating choices and getting regular exercise can greatly mitigate health risks. Prevention can greatly reduce the onset of conditions that create a need for more extensive care, including diabetes, heart disease, tooth decay and even some forms of cancer. SBNC stresses prevention education at all levels of care for its patients. In calendar year 2012, Santa Barbara Neighborhood Clinics served a total of 16,743 unduplicated patients with 81,151 total patient and health education visits (includes 35,809 unduplicated medical visits and 12,634 dental visits). Medical preventive services include health education delivered through our Health Promotion Department. The Health Promotion Department provides free, bilingual health education classes and trainings, either independently or with various partners, including Sansum Diabetes Research Institute. Class topics include asthma management, prediabetes and diabetes prevention and management classes, self-esteem, sex education, smoking cessation, stress management, teen health advocacy, Hispanic women’s health advocacy and many more. The dental education program is very innovative and unique for the Santa Barbara community, and it is a key factor in the long-term oral and overall health of patients. Our Eastside Family Dental Clinic (EFDC) staff and volunteers provide oral hygiene education and dental supplies to every single child and every member of his/her family who visits the clinic. EFDC collaborates with the Assistance League of Santa Barbara to deliver dental hygiene and nutrition education, and oral hygiene kits. Health Promotion Department 915 N. By collaborating with numerous non-profit and government organizations each year, SBNC leverages each dollar to ensure our patients receive the best possible care. Sansum Clinic is one of our partners that go above and beyond to help our patients receive specialty care which our patients can’t afford. Free to our patients, they provide radiology services such as CT scans, mammograms, ultrasounds and MRI’s. In addition, they provide free colonoscopies for patients in need. A healthy community relies on providers working together and SBNC is grateful for its partnership with Sansum Clinic, allowing us to focus on primary medical care knowing they can assist our patients with their radiology and colonoscopy services. SBNC is part of a healthcare continuum which includes Cottage Health System, Sansum Clinic and Santa Barbara County Public Health Department. We collaborate with the County on our healthcare services for the homeless and we partner with Cottage Health Systems on a variety of projects. Critical to our success are a number of non-profit Community Based Organizations (CBOs) who provide us with gifts in kind and referral services to our patients. Among the CBOs that SBNC collaborates with are: Direct Relief International, SEE International, Lutheran Medical Center of New York, St. Cecilia Society, Transition House, Unity Shoppe, Casa Esperanza, Assistance League of Santa Barbara, United Way and Legal Aid Foundation, to name a few. It is through these partnerships that we are able to deliver quality, affordable healthcare to thousands each year. Electronic Dental Records Goes Live W ith the 2014 mandate to transfer all medical records from paper to electronic, our Eastside Family Dental Clinic (EFDC) took a huge leap forward and was the first of our clinics to implement the new system. After transferring x-rays from our old Dentrix system, creating templates, training, making sure Electronic Practice Management can communicate with Electronic Dental Records, testing, etc., on May 29, 2012 our dental clinic went “live” with a system called QSI. “It was very time consuming,” said Dr. Quynh Nguyen, Chief Dental Officer. “However, the staff worked hard and we succeeded from the very start in becoming more efficient than when we were using paper charts.“ The EFDC now sees an additional 8-10 patients per day due to the increase in efficiencies brought about by the new electronic system. “We were seeing approximately 45 patients a day and now Page 8 we can see about 55. This allows us instant access to our patients’ records and reduces charting time, thus allowing us to provide care for more patients,” said Dr. Nguyen. All patient paper charts are in secured storage as required and back-up systems are in place for all electronic records.
