If you cannot attend an event, you can go to LabCorp, but be aware—not all LabCorp locations perform the required blood pressure and BMI measurements. If you go to a LabCorp that does not do BMI measurements, you will need to have your BMI completed by your physician and submit a physician-signed BMI Certification form to USPM. Completing a biometric screening will provide you with important information about your BMI, cholesterol, blood pressure and glucose level. These are four key risk indicators for major illnesses. By knowing your risk factors, you can take action to reduce your chances of developing heart disease, diabetes, and other major illnesses. Public Employees’ Benefits Program PEBP and U.S. Preventive Medicine will hold a series of worksite biometric screening events through mid-May. 901 South Stewart Street Suite 1001 Carson City, NV 89701 Biometric Screening Events Public Employees’ Benefits Program NVision Health & Wellness Program A health and wellness program for participants and their dependents (ages 18 and older) enrolled in the: Consumer Driven Health Plan (CDHP) Health Plan of Nevada (HPN) Hometown Health Plan (HHP) U.S. Preventive Medicine 877-800-8144 [email protected] Know Your Numbers CHOLESTROL Waist Size BLOOD SUGAR HDL & LDL Blood Sugar TRIGLYCERIDES Blood Pressure KNOW YOUR NUMBERS KEY NUMBERS Body Weight Blood Pressure Health Predictors LIPID PROFILE Body Mass Index TRIGLYCERIDES Know Your Numbers Health Risk Predictor NVision Health & Wellness Program What’s new with the NVision Health & Wellness Program Exciting Changes for the NVision Health & Wellness Program in 2015 PEBP has redesigned the NVision Health & Wellness Program, making it easier for you to participate and earn an incentive.
