The introduction of diagnosis-based Medicare managed care payment in 2000 (Hierarchical Condition Categories (HCC)) and its subsequent adoption by other payers including Medicaid and now Health Insurance Exchanges (HIXs), calls attention to the critical need for reporting and measuring patient severity of illness, risk of mortality and quality outcomes. HCCs have been used as the platform from which ICD-9-CM coded data impacts all of the above. This session will present detailed information regarding the Medicare HCC model as well as current future adaptations by other insurers. Important topics to be covered include the fundamentals of HCC payment methodology, data reporting requirements and impact on providers, insurers and health information professionals. At the conclusion of this program, participants will gain an understanding of hierarchical condition categories (HCCs) and risk adjustment payment model.
