Cindy Baumgartner has been a

training hundreds of food service
professionals. She has experience
as a school food services director, a
Family and Consumer Sciences
teacher, and is currently employed
as a Human Sciences Specialist in
Nutrition and Health for Iowa State
University Extension and Outreach, serving several
counties in eastern Iowa. Cindy received a BA
degree in Vocational Home Economics Education
(Foods & Nutrition emphasis) and an MAE degree in
Educational Psychology (Development and
Learning) from the University of Northern Iowa.

The $150.00 registration fee includes:

Exam administration & proctoring
Postage costs to mail certificate
(6th edition-Updated with 2013 FDA Food Code)
► If you belong to the IA Restaurant
Association, the registration fee is
$100.00, with verification of membership
at time of registration
► All participants must successfully pass the
exam with a score of 75% or higher to be certified by
the National Restaurant Association Educational
Foundation. Certification is valid for five years &
provides the CFPM (Certified Food Protection
Manager) status, now required by the IA Food Code.
► PHOTO ID is required by the National
Restaurant Association Educational Foundation. It
will be collected before the exam & returned upon
exam completion.
► You have a responsibility to get your

ServSafe Manager Manual ahead of time & to
study before coming to class. You can either:
1)
Pick up a book ahead of time at either the
Dubuque or Delaware Co. ISU Extension
Offices- once you have paid. Your regist. form
& payment can also be taken to either office.

Have the book mailed to you. Include an
additional $6.00 per book to cover mailing
costs.

is based on ServSafe
concepts and is designed to educate
food handlers on food safety basics.
