Outreach Community Health Centers pro‐ vide an array of Health Care services for Milwaukee County residents, particularly those who are under or uninsured, using a Patient Centered Medical Home model. Outreach also has Supportive Programs for those who are homeless or at risk of home‐ lessness; those suffering from mental ill‐ ness and/or substance abuse problems and those with HIV or at high risk for HIV. Primary Care: Outpatient health care for all age groups. Services include: Routine Care, Physicals, Immunizations, Preventive Care, and Diabetes and Chronic Disease Management. Family Services: Includes Primary Care, Pre‐ Natal Care Coordination and Child Care Coordination Services. Behavioral Health Services: Mental Health and Substance Abuse outpatient treatment for all ages. Services include evaluation, individual and group therapy and medication management. Dental Services: Urgent and restorative dental care. Pharmacy: On‐site pharmacy for current clients , including low‐cost options for uninsured. For those with mental illness and substance abuse disorders, Community Support Program (CSP): An assertive community treatment program model that assists adults with serious and persistent mental illness, who are homeless or at risk of homelessness, to live independently. Assists individuals with housing; obtaining mental health or substance abuse services; budgeting and access to medical and dental services. Comprehensive Community Services (CCS): A comprehensive case management program that provides clients with an opportunity to select from an array of services ranging from case management to primary care support to assist in meeting their overall behavioral, physical and social needs. Targeted Case Management (TCM): Supports individuals with severe and persistent mental illness to live independently in the community. Assistance is provided with housing; legal services; medical, mental health and substance abuse services; money management; employment and training; benefit advocacy; social network development; activities of daily living and crisis intervention. Projects for Assistance in the Transition from Homelessness (PATH): An outreach and case management program for those experiencing both homelessness and mental illness. Helps consumers gain access to different services and programs that will help them in the transition from homelessness to sustainable housing. SSI/SDI Outreach, Access and Recovery Program (SOAR): An initiative focused on assisting eligible homeless individuals in applying for SSI and SSDI benefits. For those who are homeless, Families Assisted in Transitional Housing (FAITH) and Family Abodes: Transitional housing programs for homeless adults and their children. Participants contribute 30% of their income toward their housing expense. Traditional Outreach: Assists individuals in finding shelter or housing. Helps clients to obtain identification, retrieve birth certificates, claim SSI benefits and gain referrals for medical services. Clothing, sleeping bags, hygiene kits and food are also provided as needed. Milwaukee Collaborative Supportive Services Project (MCSSP): A collaborative project to provide qualified homeless individuals who meet program requirements with an array of services ranging from housing to behavioral and physical health care needs. For those with HIV or at high risk for HIV, HIV Rapid Test: A method of testing for HIV available at all OCHC funded primary care clinics. The Rapid Response Test provides results in 20 minutes. The tester provides counseling and education related to HIV . Linkages to Care: This program is part of the Special Project of National Significance (SPNS) initiative to identify and link people living with or at high risk of HIV to appropriate levels of care.
