1st Term 2015
Programs and competitions also run in terms 2&3
MCC Hockey are running multi-age introduction
& skills programs for children in Term 1 2015.
HookIn2Hockey
Ages 5 to 11 years
Our program introduces children to the skills involved in
hockey in a safe, non-competitive atmosphere with the
emphasis on involvement, skill development & fun.
‘Game ready’ program for children new to hockey
Will help children learn and develop the basic skills to be
ready to play in Term 2&3 under 12, under 14 and under
16 competitions
When:
Saturday mornings (excluding long weekends)
9.00AM to 10.00AM
First Day: Saturday 31st January 2015
until 21st March (not 7/3), Term 2 starts 2nd May
Where:
MCC Hockey’s ground at
Melbourne High School, South Yarra
Enter via Forest Hill (off Chapel Street) Mel Ref 2L J4
By Tram: Chapel St - Tram #78 and #79
By Train: 100 metres to South Yarra Train Station
By Car:
Limited onsite parking in the school grounds or paid parking at Vogue Plaza
Cost:
Term 1 $45 ($10 disc for 2nd&3rd child in family, no fee 4th +); Terms 2&3 $tba
$30 intro pack with hockey stick, ball, shin guards, mouth guard (compulsory to wear) and singlet
Trial:
We offer a "free" one week trial where
your child will be loaned a stick & shin
guards. We will also provide a
mouthguard to keep.
Conduct of clinics on behalf of MCC Hockey:
Our coaches include qualified and registered Physical Education
teachers, with extensive hockey playing and coaching backgrounds
and a range of experiences in Primary and Secondary teaching and running these programs.

PARENT/ GUARDIAN CONSENT NEEDED TO PARTICIPATE

In allowing my child to participate in the Hockey Australia (“HA”) Hook in2 Hockey program (“the Program”) run by the MCC Hockey
Section Inc (“the club”):
· I ACKNOWLEDGE that there are inherent dangers associated with the Program which may result in my child being injured.
· To the full extent permitted by law, I agree both on behalf of the child and in my own right to ABSOLVE AND INDEMNIFY HA and the club
from any and all liability for injury, loss or damage however caused arising out of my child’s participation in the Program.
· I agree both on behalf of my child and in my own right to RELEASE AND FOREVER DISCHARGE HA and the club from all claims that I or the
child may have or may have had but for this release arising from my child’s participation in the Program.
· I AUTHORISE registered Program Coordinators to arrange medical or hospital treatment (including, without limitation, ambulance
transportation) if I am not available to do so and I INDEMNIFY HA and the club for all costs associated therewith.
· I AGREE that HA and the club retains the right to utilise images of my participation in the Program for the purposes of promoting HA’s
national recruitment program and for club recruitment purposes.
· I have read, understood, acknowledge and agree to the above declaration including the warning, release and indemnity.

HA requires the information requested in this form for use in relation to the Program. Any personal information provided will only be used in
accordance with the objects and purposes of HA, HA general business and in relation to HA’s national recruitment program. If the requested
personal information is not provided you may not be able to receive the benefits of registration in the Program. Individuals will be able to
access their personal information through HA upon reasonable notice. The club collects and uses this information in accordance with our
privacy policy as posted on www.mcchockey.org.

accident cover will only apply to participants in intra club or HV competitions in Term 2&3)
