Effective communication is a critical component of patient safety and, within a busy
labor and delivery unit, the inability for
obstetricians and staff to communicate successfully can sometimes lead to catastrophic
results. Communication could be hindered
by a difference in training or perspectives on
clinical management. Effective communication is respectful, clear, to-the-point, and unambiguous.
So, what can be done to improve communication and teamwork among obstetricians
and the labor and delivery staff they work
with each day? This question has been a focus of the Medical Assurance Company of
Mississippi’s Risk Management Staff for the
past year, and this program is a culmination
of looking for the answer.
Through presentations and nationally recognized speakers, we hope you will gain a new
perspective and a few ideas of how to build
up the team you work with each day. We will
discuss emergencies in the labor and delivery
area and how an OB Response Team might
handle these situations. During a case presentation, one of our insured obstetricians
will share an emergency situation that occurred in his practice and will be joined by
members of the team that he worked with
in the hospital and by the legal professionals
who supported him during the trial process.
The goal of all obstetrical care is a delivery
resulting in a healthy mother and a healthy
baby. Communicating and working together
as a team are two ways to strive for that goal.

UPDATE ON CURRENT OB CLINICAL ISSUES
The practice of obstetrics and gynecology is changing every
day and these changes can bring excitement, worry, challenges and anxiety to ob-gyns and the labor and delivery
nursing staff. So, what is new and important with the practice of obstetrics? In this presentation, Dr. Macones will ad-
dress the issue of contemporary labor management and the
role of delayed cord clamping, practices which could potentially reduce negative outcomes. The practice of obstetrics is
full of change - this presentation will provide some insight
into the future now.

Dr. Macones is an internationally recognized expert in perinatal clinical research and has held RO1 grants
from NICHD, NHLBI, and NCI. He has over 100 peer-reviewed publications related to clinical research in
obstetrics. His research interests include general obstetrical issues (labor induction, VBAC) and the prediction and prevention of preterm birth. He has served on numerous national committees, including the FDA’s Advisory Committee
on Reproductive Drugs and on several study sections for NIH. He has worked closely with ACOG, most recently as Vice Chair for
the Committee on Obstetrical Practice Bulletins. He currently serves on the Board of Directors for the Society for Maternal-Fetal
Medicine, is Associate Editor of the American Journal of Obstetrics and Gynecology, and is Co-Chair for the ACOG Task Force for
PROLOG Obstetrics.

THE BENEFITS OF USING A RAPID RESPONSE TEAM IN AN OBSTETRICAL CRISIS
An obstetrical emergency in a labor and delivery setting can
challenge even the most experienced physicians and staff.
It is an unexpected event that could lead to negative consequences or even maternal and fetal death. The development
of a multidisciplinary obstetrics Rapid Response Team will
often prove life-saving and beneficial to the management
of the crisis. A Rapid Response Team offers a standard reaction to the emergent event. Attendees will receive current
evidence-based information regarding statistics of successful versus non-successful obstetrical Rapid Response Teams.

PREVENTING SEVERE MATERNAL MORBIDITY AND MORTALITY
Patient care emergencies may occur at any time and in any
setting and often result in untoward outcomes. In this presentation, Dr. Martin will review the current physician-nurse
management of obstetric emergencies, specifically involving hypertension, hemorrhage and embolism, relative to
the essential patient safety care issues included in current
maternal-perinatal bundles for hospitals. Dr. Martin will dis-

Carol Curran has been a practitioner in high-risk obstetrics for over 25 years. Her experience spans as a clinical
staff nurse to Ob-Gyn nurse practitioner and consultant from the West Coast of Los Angeles, California, to the
East Coast of Virginia Beach, Virginia.
Ms. Curran began her long career in tertiary care settings at the Medical College of Virginia in Richmond,
Virginia, where she gained knowledge, skill, and expertise with high-risk obstetrical patients requiring arterial
lines, Swans-Ganz catheters, and ventilator support.
Ms. Curran is the senior educator, perinatal specialist, and business development specialist at LAMMICO, Medical Interactive Community. She is published in JOGNN, JOPNN, AJMCN, AWHONN’s Perinatal Nursing Textbook (3rd Ed), and is the co-author of
abcdEFM The Textbook & Pocket Guide: Electronic Fetal Monitoring. She is recognized as a national and international perinatal
educator and medical-legal expert.

INTRAPARTUM FETAL HEART RATE MONITORING: APPLYING PRINCIPLES OF PATIENT
SAFETY
In 1999, a highly publicized report from the Institute of
Medicine identified major deficiencies in the United States
health care system, which fueled the rapid growth of the
modern patient safety movement. One of the greatest risks
to patient safety in obstetrics is poor communication of
electronic fetal heart rate monitoring findings. Standardiza-
tion and elimination of unnecessary complexity are two of
the cornerstones of improved patient safety. In this presentation, you will learn a standardized, simplified approach to
the definition, interpretation, and management of electronic
fetal heart rate monitoring that is evidence-based and reflects consensus in the literature.

treat severe maternal postpartum hemorrhage; offer recommendations to recognize amniotic fluid embolus and start
emergency care; and review guidelines for the prevention of

Recognized not only in his home state of Mississippi through his practice of maternal-fetal medicine at the
University of Mississippi Medical Center, Dr. Martin is also a nationally-recognized leader and speaker on the
specialty of obstetrics and gynecology. A former President of ACOG, Dr. Martin has served on the International Federation of Gynecology and Obstetrics (FIGO) Executive Board, representing ACOG and the US. In addition, he is a past
president of the North American Society for the Study of Hypertension in Pregnancy and the Society for Maternal-Fetal Medicine.
Dr. Martin has more than 600 scientific publications and communications of various types to his credit over the past 41 years. Most
recently, Dr. Martin has been published in the area of obstetric management of patients with hypertensive and hematologic disorders
of pregnancy, especially preeclampsia, eclampsia and HELLP syndrome. Dr. Martin continues to be an active clinician, educator,
administrator and investigator with a primary focus of interest in hypertensive complications of pregnancy.

THE POWER OF TEAMWORK IN DEFENDING AN OBSTETRICAL MALPRACTICE CASE
This case from the MACM files is an example of an unexpected clinical emergency that occurred and could have
compromised the safety of the mother and/or the baby. The
MACM insured obstetrician will review this case, along with
members of the legal team involved in defending him at
trial. In addition, you will hear from the expert witness and
the labor and delivery nurse to gain their perspectives of the
situation.

Professor of Clinical Obstetrics, Gynecology and Pediatrics, Keck School of Medicine
University of Southern California
In addition to his varied academic, research and medical practice responsibilities, Dr. Miller is the medical
director of the Children’s Hospital, Los Angeles-University of Southern California Institute for Maternal-Fetal
Health. The institute serves as a multidisciplinary, multi-institutional center specializing in fetal and neonatal
diagnosis and therapy. Dr. Miller is also director of maternal fetal medicine and director of the USC Obstetrics
and Gynecology Residency Program Rotation at Hollywood Presbyterian Medical Center in Los Angeles. Dr.
Miller has served as a peer reviewer for a number of noted medical journals that focus on obstetrics and gynecology and he is the
co-author of the text, “Fetal Monitoring and Assessment”, 6th edition.
