Este directorio brinda una lista de proveedores que forman parte de la red de Blue Cross
Medicare AdvantageSM.
Este directorio es para la red completa de proveedores de Blue Cross Medicare Advantage que
se ofrece a las personas elegibles de Medicare y que residen en los condados de Collin, Dallas,
Denton y Tarrant.
Este directorio tendrá vigencia a partir de octubre de 2015. Es posible que se hayan agregado

directorio. No garantizamos que cada proveedor siga aceptando miembros nuevos. Para obtener

Sección 1: Introducción
Este directorio brinda una lista de proveedores que forman parte de la red de Blue Cross Medicare Advantage.
Para obtener información detallada sobre su cobertura de atención médica, consulte su Evidencia de cobertura.
Blue Cross Medicare Advantage es un plan de una organización de proveedores participantes (Preferred Provider
Organization, PPO). Todos los proveedores del plan Blue Cross Medicare Advantage se encuentran disponibles
para los asegurados a través de autoderivación, con excepción de los servicios que requieren autorización previa.
Los “proveedores que forman parte de la red” que figuran en este directorio han acordado brindarle cobertura
de atención médica. Usted puede acudir a cualquiera de los proveedores que forman parte de la red que figuran
en este directorio. Si ha estado consultando con un proveedor que forma parte de la red, no se le exige que
siga consultando al mismo proveedor. En algunos casos, es posible que reciba servicios cubiertos por el plan de
proveedores fuera de la red. Si consulta con proveedores que forman parte de la red de Blue Cross Medicare
Advantage, sus copagos y/o coseguro serán menores que si visita proveedores fuera de la red.
No necesita autorización previa para buscar ayuda médica en una situación urgente o de emergencia.

Nunca deberá pagar a ningún proveedor que no forma parte de la red una suma superior a la que el proveedor
está autorizado por Medicare. Solicite al proveedor que no forma parte de la red que nos facture a nosotros
primero. Si usted ya pagó los servicios cubiertos por el plan, le reembolsaremos nuestra parte del costo. Si
recibe una factura por estos servicios, puede enviarnos la factura para que la paguemos. Pagaremos a su
proveedor que no forma parte de la red nuestra parte de la factura y le haremos saber la cantidad que tiene que
pagar usted, si corresponde.
Servicios de atención médica de emergencia
Los servicios de atención de emergencia son servicios de atención médica prestados en un hospital o centro
similar para evaluar y estabilizar problemas de salud de inicio reciente y graves, que incluyen, entre otros, dolor
intenso que ocasionaría que una persona prudente con conocimiento promedio de medicina y salud considere
que la afección, enfermedad o lesión es de una magnitud tal que la falta de atención médica inmediata podría
representar:
• Peligro grave para la salud del paciente.
• Discapacidad grave de las funciones corporales.
• Disfunción grave de cualquier órgano o parte del cuerpo.
• Desfiguración grave.
• Peligro grave para la salud de un feto en caso de que la paciente esté embarazada.
Los servicios de atención médica de emergencia necesarios para evaluar y estabilizar un problema de salud de
emergencia están cubiertos por Blue Cross Medicare Advantage. A los asegurados que sufran un problema
de salud de emergencia se les aconseja visitar al proveedor de emergencia más cercano. La evaluación y
estabilización de un problema de salud de emergencia en un hospital o centro similar no requiere certificación
previa. Los servicios de atención médica de emergencia estarán cubiertos al nivel de beneficios que forman
parte de la red. Con excepción de la atención médica de emergencias o urgencia, es posible que sea más
costoso recibir atención por parte de proveedores que no forman parte de la red.

Servicios de diálisis renal fuera del área
Los asegurados podrán recibir servicios de diálisis necesarios por motivos médicos por parte de cualquier
proveedor médico/profesional calificado que el asegurado elija cuando se encuentre temporalmente fuera del
área de servicio de Blue Cross Medicare Advantage y no pueda acceder de manera razonable a los proveedores
médicos/profesionales de diálisis u otros proveedores de Blue Cross Medicare Advantage. No se necesita aviso
ni certificación previa. Sin embargo, es posible que un asegurado avise voluntariamente a Blue Cross Medicare
Advantage si se encontrará temporalmente fuera del área de servicio. Es posible que Blue Cross Medicare
Advantage brinde asistencia al asegurado para ubicar proveedores médicos/profesionales de diálisis calificados.
Con excepción de la atención médica de emergencias o urgencia, es posible que sea más costoso recibir
atención por parte de proveedores que no forman parte de la red.
