Each nominee will be recognized at the Annual Star of Life Award Banquet to be held April 1 . At the OKAMA Stars Banquet each star will receive a Star of Life trophy, a Star of Life pin, and a copy of the video that will be shown during the event. The nominees will also be considered for an all expense paid trip to Washington D.C. The chosen nominees will represent Oklahoma at the National Star of Life ceremony presented by the American Ambulance Association. Two nominees, one from a rural EMS agency and one from a suburban EMS agency may be picked by the OKAMA Board to represent Oklahoma and OKAMA. Eligible nominees include all levels of EMT, billing personnel, dispatchers, and even medical directors. If you have an employee that has had an outstanding “save”, goes above and beyond what is required, or maybe just helps to further the mission of EMS, nominate them. The nomination will show your employees that you appreciate the things they do for your agency. It also will show the community what a great service you have and the dedicated employees that serve them. Oklahoma Ambulance Association members participate in the Star of Life program at no charge. Non-member services are encouraged to go to the website www.okama.org and join. Non-members may also participate, however a fee will be assessed to cover the cost of the Star’s participation. OKAMA will present each nominee with a commemorative Star of Life trophy and lapel pin just for being nominated during the program on April 1, 2015. We also produce a special DVD power point that will outline the facts you provide on your nominee, of which a copy will also be provided. We ask that you send us 8 good quality photographs large size 2-5 mb (at least 2mb) no “selfies”, with the nomination form. You should consider backgrounds of contrast or action shots of your nominee doing job related tasks, i.e. checking equipment, taking vitals, dispatching, looking at maps. The program is a dress-formal event with coat and tie recommended (or service dress uniform). The OKAMA nomination committee will review all applications received by the February, 2015 deadline date and will make a decision as to which nominees will receive an all- expense paid trip to Washington, DC where they will represent Oklahoma at the American Ambulance Association Star of Life ceremony April 1315, 2015.
