The Center will soon be embarking on a BIG renovation incorporating Universal Design throughout the building. The goal of Universal Design is to produce spaces and objects that are designed for everyone to use – no matter their level of ability. No exclusions. The doors will be wider so members who use wheelchairs can easily and independently navigate throughout the building. The front doors will open automatically. The REACH Adult Day Program is a year-round day program for adults who work part-time, are unemployed or who want something different from the traditional workshop environment. Members must have graduated from high school or be at least 22 years of age to enroll. REACH gives members opportunities to continue to learn, make new friends and succeed while having meaningful experiences throughout the day. The program is offered five days a week, Monday through Friday, from 10:00 a.m. until 3:00 p.m. Members choose the number of days they wish to attend, as well as having the option to come in at 9:00 AM to begin their day for an extended day. REACH members partner with staff in planning activities and classes for each month. Some member’s choices have included fitness activities, social groups, pet therapy, swimming, fishing, horticulture, drama and art. REACH members can also choose from other classes that members express interest in such as Shop ‘N’ Cook, Janitorial Training, Book Club and learning basic computer skills. REACH offers a Relationship Class taught by a licensed clinical counselor where members are able to discuss issues such as friendships, dating and learning to advocate for themselves. Illinois Imagines, a sexual assault prevention and education program is also offered. Through a generous grant from Macy's, The Center has been able to enhance and adapt the curriculum to better meet the needs of REACH members. REACH collaborates with organizations such as Arts of Life, Our Place of New Trier and Keshet where members engage in a variety of activities, including exploring the community, making new friends and reconnecting with old friends. For those individuals who are interested in gaining work skills, REACH offers vocational skills training. The purpose is to provide training and support in an area of interest which, in the future, may enable the individual to pursue community employment in this field. Job skill areas include, but are not limited to, training in shredding and recycling including collecting, sorting and going out to the recycling center in the community to earn cash for future outings of the participants’ choice. REACH is committed to providing members opportunities to connect to the community with more than 20 partners. REACH members are currently involved in projects at: Lincoln Park Zoo, Adopt-a-Highway Department, Horsefeathers Therapeutic Riding Stables, Marriot Hotels, Home Delivered Meals, Catholic Charities Food Pantry, Bernie’s Book Bank and Whitehall of Deerfield. The Center is committed to working with each individual to find the program that best meets his/her needs. The process is as follows: • CEL Staff and the prospective REACH member meet and get to know each other during an initial interview • Staff and/or veteran REACH members host a tour of The Center • A 30-day assessment period at the start of the program enables the participant and the REACH staff to gather the tools they need to develop his/her personal REACH for Success Plan. The plan incorporates the individual’s dreams, desires and interests to establish a course of action to achieve his/her individual goals. Progress toward the goals will be tracked by the REACH member, with assistance from staff. A quarterly progress report will be shared with key people in the member’s life. • A primary outcome of the program is for the REACH member to make progress towards his/her personal goals. • We require that members make a minimum six month commitment to the program. If you are interested in learning more about the REACH day program contact Sierra Shum at 847.315.9920 or complete the interest form on page 30. We will contact you to arrange a tour. 29 Reach interest form Date: Prospective Member Name:D.O.B: Address: Guardian(s):Relationship(s): Phone (Home):Phone (cell): Email Address: I AM INTERESTED IN THE FOLLOWING DAYS: (Check all that apply) __Monday __Tuesday __Wednesday __Thursday __Friday I WOULD LIKE TO START PROGRAM ON THIS DATE: I AM MOST INTERESTED IN: (Check all that apply) ___Pre-Employment ___Horticulture Training ___Leisure/Hobbies ___Job Placement ___Independent ___Volunteer Work Living Skills ___Social Groups ___Continuing Education ___Community ___Healthy Living Skills Experiences ___Creative Arts MY LEVEL OF INDEPENDENCE IN PERSONAL CARE IS: ___Minimal Assistance ___Independent ___Complete Assistance ___Verbal Prompts I AM CURRENTLY: ___Unemployed ___Volunteering ___Attending a Workshop ___Working in the Community (Part-Time or Full Time, circle one) ___In School Location________________ I COMMUNICATE: (Check all that apply) ___Verbally ___Using Pictures ___Using an Adaptive Device ___Using Sign Language ___Using Gestures ___Can Read and Write___Other I HAVE MEDICAL OR BEHAVIORAL NEEDS THAT REQUIRE 1:1 SUPPORT THROUGHOUT THE DAY: ___No ___Yes (If yes, please list) I HAVE TRANSPORTATION TO/FROM REACH: ___Yes ___No ANY ADDITIONAL INFORMATION: Name of Person Completing Form:Relationship: The Center for Enriched Living, 280 Saunders Road, Riverwoods, Illinois Phone: 847.948.7001 Fax: 847.948.7621 www.CenterForEnrichedLiving.org 30 registration 31 registration information Fees and Registration 1.
