Starting January 1, 2017, there will be changes to your health and dental benefits. The
32BJ Health Fund sent you a letter in November with all of the details – and we are
providing the information again here because it is so important that you understand
your benefits and use them to get high-quality health care for yourself and your family.

Dental plan changes
Two changes to your dental plan also take effect on January 1. First, Delta Dental will
now be providing your dental benefits instead of Administrative Services Only. Your
new network of high-quality dentists is called the Delta Dental NYPPO network, and
it is larger than the network you have had. Second, there are some changes to your
out-of-pocket costs for dental care (see chart).

Cambios en el plan dental
Verá dos cambios en su plan dental el 1 de enero. En primer lugar, su proveedor de

at a 5 Star Center and the care you get is
top of the line. At a 5 Star Center, you get:
1. Attentive care. You have a doctor who
takes the time to know you. Regular
checkups with a doctor who knows
you and your medical history is the
best way to catch problems early when
they’re easier to treat. At a 5 Star Center,
you also have your own team of caring
health-care professionals to help you
manage your care and your health. 5 Star
Centers emphasize patient-centered
primary care and all are rated at the
highest quality level by the National
Committee for Quality Assurance.
2. Easy access. Most 5 Star Centers
have evening and weekend hours to
make it easier for you to get care. If
you are already a patient, same day
appointments are available when you
are sick and cannot wait – and regular
checkups and follow-up appointments
are easy to schedule. And you can call
anytime if you are sick and need advice.
3. The 5 Star Wellness Program. If you
have one of these chronic conditions –
asthma, COPD, diabetes, heart disease,
hypertension, PAD – or you have had a
stroke, you can join the 5 Star Wellness
Program to get extra help staying your
healthiest. When you sign up for the
program, you commit to getting all of
your primary care at your 5 Star Center
and you receive:
