Pretty soon your child/children will be heading back to school.

forms. This is a perfect time to make sure they get a full
check-up as well. Your health plan will pay for this
visit.
At this time
CHILD CHECK-UP and SHOTS – Call Your Doctor
Check-ups help find health issues before they become
bad. Child check-ups are done when your child is 1
month, 2 months, 6 months, 12 months, 15 months and
18 months. Starting when your child is 2 years old a check-up is done one
time each year.
LEAD TESTING – Call your Doctor
Children should be tested for lead exposure at 1 and 2 years of age.
DENTAL EXAM – Call Dentaquest at 1-800-964-7811
Children should see a dentist for the first time when they turn 1 year old.
