Attending a worksite event is the
easiest way to complete your
biometric screening (labs, blood
pressure, and BMI measurement). If
you cannot attend an event, you can
go to LabCorp, but be aware—not
all LabCorp locations perform the
required blood pressure and BMI
measurements.
If you go to a LabCorp that does not
do BMI measurements, you will
need to have your BMI completed
by your physician and submit a
physician-signed BMI Certification
form to USPM.
Completing a biometric screening
will provide you with important
information about your BMI,
cholesterol, blood pressure and
glucose level. These are four key
risk indicators for major illnesses.
By knowing your risk factors, you
can take action to reduce your
chances of developing heart disease,
diabetes, and other major illnesses.

PEBP has redesigned the NVision
Health & Wellness Program, making it easier for you to participate
and earn an incentive. The changes
you will see in 2015 include:

Removal of the CDHP and HMO
on-line benefit tutorials

Removal of the 3 physical
activity challenges for those with
a BMI equal to or greater than 26

Only five steps required to earn
an incentive

New wellness program website
that is easier to use and navigate
Complete Five Easy Steps
by May 31, 2015
1. Complete new registration at
NVision.PEBP.state.nv.us by May 31, 2015
2. Complete the Health and Well-being
Assessment (HWA) by answering questions
related to lifestyle habits (like smoking, seat
belt use, and exercise)
3. Schedule and complete your Biometric
Screening (biometric screening includes lab
work, cotinine/tobacco use testing, body
mass index (BMI), and blood pressure
reading
4. Complete a preventive exam visit with your
physician, enter the date of the visit and your
physician’s NPI number (available at http://
npibd.org) into your NVision account
5. Complete a preventive dental visit/teeth
cleaning and enter your dentist’s NPI
number (available at http://npidb.org) into
your NVision account

Wellness Incentive
CDHP and HMO primary participants
who complete the required steps by May
31, 2015 will receive the following
incentive beginning July 1, 2015:

CDHP primary members will receive
a $50 monthly HSA or HRA
contribution.

HMO primary members will receive
a $50 monthly health insurance
premium reduction
