Registration – Please note that both Part 1 AND Part 2 of the registration forms need to be completed.
Payment Options
The following options are available for paying fees:
Cheque:
Make cheques payable to Wanneroo Lacrosse Club
Money/Postal Order:
Make payable to Wanneroo Lacrosse Club
Direct Deposit:
Contact your bank to transfer funds to St George BSB: 116-879 Act No: 475678405 (Family name as
the reference note) Please keep transfer receipt as proof of payment.
KidSport Voucher:
Obtain approval from your local council to pay for fees using a KidSport Voucher. (Conditions apply
– see your local council website for details)
EFTPOS:
We accept Visa or Mastercard (no Amex or Diners). Or you can pay using your cheque or savings
account.
Payments/completed registration forms may be posted to PO Box 391 Greenwood WA 6924. If paying by direct deposit or
KidSport, completed registration forms may be emailed to [email protected]. If you have difficulty in paying at
this time of year and you are not eligible for Kidsport, please speak to one of the club representatives to arrange a payment
schedule or to make alternative arrangements. We want you and/or your child playing our great sport; however please note
that un-financial members will not be permitted to play beyond the first few games of the season. Those members attending
the AGM will receive a 10% discount on fees only. A single payment may be made for multiple players, but a separate
registration form must be received for each player.
Disclaimer
The player/parent/guardian signing the declaration overleaf acknowledges and agrees to the following:
I acknowledge that playing, coaching, officiating or participating in any capacity in a Lacrosse, Modcrosse or Sofcrosse game
carries with it the risk of personal injury. To the extent permitted by law, I agree both on behalf of my child and myself and in
my own right to ABSLOVE and INDEMNIFY the Lacrosse Association of Western Australia (LAWA), its member clubs and
associations, their members, officials, coaches, referees and associated sponsors from any and all liability, loss or damage
however caused (whether by negligence or some other event) arising out of my or my child’s participation in Lacrosse,
Modcrosse and/or Sofcrosse games and training for such games. I agree on behalf of my child and myself and in my own right to
RELEASE and FOREVER DISCHARGE the LAWA, its member clubs and associations, their members, officials, coaches, referees and
associated sponsors from all and any claims that I and/or my child may have had but for this release arising from my and/or my
child’s participation in said games and training for said games. I AUTHORISE duly appointed LAWA officers or duly appointed
member club officials to arrange medical and/or hospital treatment (including without limitation ambulance transport) if I am
not able or available to do so myself and I INDEMNIFY LAWA, its officers and member clubs and associations, their members,
officials, coaches, referees and associated sponsors for all costs associated therewith.
Privacy Policy
Personal information collected by Wanneroo Lacrosse Club is for the primary purpose of the membership requirements and
competition purposes. It will not be released for any commercial gain and will be maintained in a secure environment as
required by the Privacy Act. Completion and lodgement of the registration form overleaf indicates acceptance of the club’s
policy.
Photographic Permission
From time to time, photos from games and group or team photos may be taken for club purposes. These may be used for
general publicity, when seeking grants from council and/or government authorities, included in the newsletter or maybe posted
on the club’s web page and/or Facebook page. In most cases, photographed players will not be identified by name. If you circle
No on the ‘Additional Details’ form, every attempt will be made to ensure your or your child’s face does not appear in these
photos. However, in signing the declaration overleaf, you acknowledge that the omission of your photo may not always be
possible or practical in a group or general photo. For example in general game shots, it is not possible to check every
registration form at the time of taking or posting the photo. If you have circled No and you notice your or your child’s image in a
photo, you may request that the photo be removed, by telephoning, emailing or writing to the President, Secretary or Registrar.
We undertake, where practical, to remove the photo as soon as possible. We thank you for your understanding and
cooperation in this regard.
