CONTESTANT INFORMATION/APPLICATION FORM – 2015 (Please review this form in great detail)
THIS EVENT IS FOR AMATEURS ONLY ALL PROFESSIONALS WILL BE DISQUALIFIED
Langley Has Talent is a project of the Rotary Club of Langley (Doing Business As) in partnership with the Rotary Clubs of Langley Central, Langley
Sunrise and Aldergrove.
Thank you for expressing an interest in applying to be a contestant in Langley Has Talent. Please read the Contestant Information Form carefully as
some sections apply to solo acts and others to group acts only. As the producer of the Program, (Rotary Clubs of Langley ((Rotary Club of Langley –
DBA “Langley Has Talent”)) in partnership with Langley Central, Langley Sunrise and Aldergrove), reserve the right, exercisable at any time in its
sole discretion, to disqualify you or any member of your act from the competition or the audition process should such individuals supply untruthful,
inaccurate or misleading personal details or information or for any other reason whatsoever. It is important to note that this project is not affiliated
with any other show, performance or company.
Applicants deemed appropriate by the pre-screening committee will be invited to audition on January 31, February 7, 14, or 21 (so please keep
these dates as clear as possible). Auditions will be approximately 90 seconds in length and will be hosted at Kwantlen Polytechnic University
(Langley Campus) and/or an alternate location which will be confirmed when audition confirmation emails are sent out. Acts/groups will be given a
specific time and location for the auditions. The Semi-Final performances that will take place in front of a ‘Live’ audience. After the auditions the
Acts/Groups chosen to move on in the process will be given a designated amount of time to perform in the Semi-Final show. This time will be given
only after the auditions have been completed and the make-up of the show is determined.
The main venue for the auditions will be Kwantlen Polytechnic University (Langley Campus – Glover Road and the Langley Bypass) and you will be
reminded of this at the time you receive your audition time (if the location of your audition is different from this, you will be notified of this at the
time you receive your audition time as already indicated). The venue for all Semi-final and Final performances will be at Christian Life Assembly

on March 28 , 2015 and April 18 , 2015 respectively. Please mark these dates down.

ALL Participants MUST ATTEND a mandatory semi-final walkthrough at 7:00 p.m. on Thursday, March 26 at Christian Life Assembly. Also, an

information meeting for participants is scheduled to take place on Thursday, February 26 in the evening (times and location to be confirmed

closer to this date). There will also be additional support given to interested participants on March 7 (between 8:30 and 4:00 at Kwantlen).
Please mark all of these dates down and/or make note so that you do not run into future time conflicts.
Qualification: Applicants will not be able to participate IF they do not live in Langley or have an appropriate connection to Langley. Note that there
is more information on this on the next page. Also, participants cannot be professional (i.e. making a living at their talent) and cannot have ever

won a 1 , 2 , or 3 place prize in previous Langley Has Talent competitions. All decisions regarding the Acts/Groups invited to audition are final
and there is NO APPEAL PROCESS.

The applications for Langley Has Talent 2015 are due by Friday, January 30 , 2015 and must be submitted via email (i.e. print, fill out, scan, and
email). The submission email address is new this year and is as follows: [email protected]. A print copy of the Registration Form
must be brought to the auditions.
It is critical that The Contestant Information and Contestant Declaration Forms be completed in detail. Failure to provide all necessary information
may result in disqualification.
Audition details: Upon receipt of your online Registration Form, acknowledgement will be sent so you know we received it. Closer to the actual
auditions, a confirmation of your audition time, location, etc. will be e-mailed to you. The auditions will be 90 seconds in length, please bring the
following:
1) Full payment in cash.
2) A print copy of the completed Registration Form signed in the correct places.
3) Whatever you need in order to complete your audition (note that a sound system and a piano will be provided).
IF you have a special request around your timing and/or availability for the auditions specifically (at this point), please indicate this in
the body of your email when you send forward your application. Note that special requests will only be accommodated if possible
and as a result, we do not guarantee we will be able to meet your request. It is always preferred that you make yourself available for
the dates indicated in this application. The organizing committee thanks you for your interest and we look forward to seeing you at

In full and complete consideration of the Producer possibly including me by a videographer or photographer for the sole
use and control of the Langley Has Talent and without any further consideration due from the Producer or any thirdparty to me, and with the understanding that the Producer, in the Producer’s sole discretion may choose whether or not
to use the captured images that include me in any promotional or revenue project in the future, I hereby agree as
follows:
I grant to the Producer and the Producer’s successors, licensees and assigns, the irrevocable right (but not the
obligations) to film, tape and/or photograph, record, exhibit, edit and otherwise use my name, likeness, voice,
conversation, sounds and biographical data on or in connection with the Event in any manner in the Producer’s sole
election and sole discretion. I agree that the Producer is the sole owner of all the results and proceeds of such film, tape
and/or photography and recording and my appearance thereon, with the right for the Producer and the Producer’s
successors, assigns and licensees forever and throughout the universe, to use the same and any portion thereof of my
name, voice, likeness and biographical material and editorial comments concerning me in any and all media, whether
now known or hereafter devised, including, without limitation, motion pictures, television and publishing, and in
connection with the advertising, sale, promotion, marketing, merchandising, distribution, publicizing and any and all
other types of exploitation of the Event or any part thereof. The Producer shall also have the unrestricted right to edit
the content of the text of the Program in any manner or form.
I hereby release, discharge and hold harmless Rotary Club of Langley [doing business as – DBA “Langley Has Talent”] in
partnership with the Rotary Club of Langley Central, Rotary Club of Langley Sunrise and Rotary Club of Aldergrove), all
television broadcasting stations, sponsors, advertising agencies and any and all others connected with the Event and
each of their related/associated companies, subsidiaries and/or affiliated companies partners and the officers, directors,
employees, volunteers, agents and representatives of each of the foregoing (collectively, the “Released Parties”), of and
from any and all claims, demands or causes of action that I may have, including without limitation, claims based upon
defamation or invasion of privacy, or any other matter arising out of or in any manner connected with the use and
exercise of the rights granted to the Producer herein.
I affirm that neither I nor anyone acting for me gave or agreed to give anything of value to any member of the
Producer’s production staff, anyone associated in any manner with the Producer’s Society/Clubs or any representative
of any television network or station for the Producer’s undertakings hereunder. I understand the Producer’s policy
prohibits such payments, and I further understand that it is an offense, unless disclosed to the Producer prior to
broadcast/performance, for any such arrangement or my acceptance or agreement to accept anything of value to
promote any product, service or venture on or off the air. I am also aware that it is an offense to use any prepared
material containing such a promotion where I know the writer received consideration for it and fail to disclose this to
the Producer prior to Event/Broadcast. I shall notify the Producer immediately if any person attempts to induce me to
violate the foregoing or to do anything dishonest.
I represent and warrant to the Producer that I am at least 19 years of age and have the full, complete and unrestricted
right and authority to enter into this agreement. I agree that my appearance in the Event/Program, if any, shall not be
deemed to be a performance and is not employment under any collective bargaining agreement or performing arts guild
and does not entitle me to wages, salary, residuals, corporate or other benefits or other compensation under any such
collective bargaining agreement or otherwise. I agree I am not a candidate for public office. If I am not at least 19 years
of age, I agree to have my parent or legal guardian sign the acknowledgement at the end of this release.
I acknowledge that I have the right to seek independent legal advice regarding any and all legal contracts/forms prior to
entering any legal agreement. Legal advice can be obtained at no cost to the Producers prior to entering any legal
agreement. This includes any minor having his/her legal guardian seeking legal advice on their behalf.
I agree not to disclose or discuss events related to the Event or authorize any third-party to do the same, without the
prior approval of the Producer’s authorized representative. I shall not directly or indirectly issue or permit the issuance
of any publicity whatsoever with respect to the Event or my appearance in connection therewith. I shall not use the
name of the Event in anyway at any time without the Producer’s prior written approval in each instance.

In addition, I agree to cooperate with the Producer in having this Agreement and any past or future agreements entered
into by the Minor in connection with the Event approved by the applicable court and I hereby waive notice and any
opportunity to appear and be heard in connection with any such proceedings.

in Producer’s sol discretion or to refrain there from. The Producer shall also have the unrestricted right to edit the
content and text of any recordings of the auditioning process and any video and sound recording and other materials I
submit in connection therewith, as well as the Event, Langley Has Talent and all film, tape, photography, recordings and
my appearances thereon, in any manner or form. I further agree that the Producer are the sole owner of and may freely
assign all results and proceeds of such film, tape, photography, recordings and my appearances thereon.
In the event I perform any original music in connection with the auditioning process or in the event, Langley Has Talent,
including musical arrangements, titles, lyrics, music and all other elements of such songs (“music”) or submit any original
photographs (“photos”), or original audio and/or video recordings (“recordings”) to the Producer, I hereby represent
and warrant that I own, control or have obtained all rights, (including, without limitation, all copyrights) in and to any
and all such music, photos and recordings, and I hereby grant to the Producer, without charge, the rights necessary to
use such music, photos and recordings on and in connection with the Event, Langley Has Talent, and the rights required
to exploit the Event and the ancillary rights therein as described anywhere in this Personal Release (including, without
limitation, in advertisements, promotions and publicity), in any and all media now known or hereafter devised, or for
any purpose, throughout the universe in perpetuity. I hereby represent and warrant that I have obtained all of the
rights, clearances, and releases necessary for the Producer to exploit said music, photos, and recordings in connection
herewith. I understand that all submissions of music, photos, recordings or other materials become property of the
Producer and will not be returned to me.
I agree to follow all of the Producer’s rules, directions and instructions in all matters relating to the Event, Langley Has
Talent, which rules are subject to change at the Producer’s sole discretion. I will accept any and all decisions of the
Producer on all matters, including, without limitation, discretionary matters (including, but not limited to, the interview,
audition process and contestant selection) as final and not subject to challenge, and acknowledge that the interests of
the Event shall override those of any participant. I agree that my selection to audition is within the Producer’s sole
discretion and that the Producer is not obligated to grant me an audition, regardless of my place in the audition process.
In the event I am granted an audition, I agree that my selection as a contestant for the Event, “Langley Has Talent,” is
within the Producer’s sole discretion and that the Producer is not obligated to select me. I acknowledge that the
Producer reserves the right, exercisable at any time in its sole discretion, to disqualify me from the Event should I at any
stage supply untruthful, inaccurate or misleading personal information, fail to abide by the rules or regulations of the
Program, or for any other reason or of no reason at all, in the Producer’s sole discretion. If I am selected to advance
beyond this initial audition, I agree that the Producer is not obligated to have me appear on, or to telecast my
appearance on, the Event, Langley Has Talent, and may disqualify me at any time and for any reason or no reason, in its
sole discretion. I understand that 100 Acts/Groups will be invited to audition from all those who make application and all
decisions are final and not subject to challenge.
I acknowledge that my audition or participation in the Event, “Langley Has Talent”, shall include my own act or
performance, which I created or selected for the Event. I understand that since this is my own act that I am solely
responsible for determining whether I am capable of performing the act in the Event Langley Has Talent; provided,
however, the Producer may, it is sole discretion, refuse to allow me to audition or participate in the Event if the
Producer reasonably believes I am not capable of performing the act, safety concerns or the Art does not fit “family”
model of performance content.
I hereby release, discharge and hold harmless Rotary Club of Langley, doing business as “Langley Has Talent”, Rotary
Club of Langley Central, Rotary Club of Langley Sunrise and Rotary Club of Aldergrove, all television broadcasting
stations, sponsors, advertising agencies and any and all others connected with the Event and each of their
related/associated companies, subsidiaries and/or affiliated companies partners and the officers, directors, employees,
volunteers, agents and representatives of each of the foregoing (collectively, the “Released Parties”), of and from any
and all liability or without limitation, the interview and audition process, the use or reuse of my appearance, name,
voice, singing voice, likeness, signature, biographical information, music, photos, and/or recordings, or the breach or
alleged breach of any provision hereof and I agree not to make any claim against Producer or the Released Parties as a
result of my participation or non-participation in the Event, Langley Has Talent, or in any aspect or phase thereof
(including, without limitation, the interview and audition process) and in connection with any use or reuse of my
appearance, name, voice, singing voice, likeness, biographical information, music, photos, and/or recordings (including,
without limitation, any claim based upon defamation or invasion of privacy and/or publicity). I hereby represent and
warrant that I have the right and authority to grant the rights granted to the Producer hereunder, that any music,
photos, and recordings that I submit to the Producer are my original works of authorship and that I am the sole owner of
such music, photos, and recordings. In addition, I agree to indemnify the Producer and the Released Parties and to hold
each of them harmless from any and all liability, claim, action, damage, expense, and loss of any kind (including, without
limitation, attorney’s fees) caused by or arising out of the Producer’s use of the music, photos, and recordings, and
arising out of any breach or alleged breach of any provision hereof or any statement, action or failure to act by me
during or in connection with my participation in the Program, its interview or audition process or otherwise.
Furthermore, I understand that in connection with the Event or in any aspect or phase thereof (including, without
limitation, the interview and audition process), I and others may reveal or relate information of a personal, private,
surprising, defamatory, disparaging, embarrassing or unfavorable nature, and that my actions and the actions of others
appearing in the Event may be embarrassing or of an otherwise unfavorable nature that may be factual or fictional. I
further understand that my appearance, depiction, and portray in connection with the Event or in any aspect or phase
thereof (including, without limitation, the interview and audition process), and my actions and actions of others in
connection with the Event or in any aspect or phase thereof (including, without limitation, the interview and audition
process), may be disparaging, defamatory, embarrassing or of an otherwise unfavorable nature, may expose me to
public ridicule, humiliation or condemnation, and may portray me in a false light. The consents, authorizations,
assumptions of risk and releases set forth in this Agreement expressly applies to all such potential harms. Without in any
way limiting the foregoing, in no event shall I have any right to seek injunctive or other equitable relief against any of the
Released Parties.
I acknowledge that there is a possibility that after my execution of this Agreement, I will discover facts or incur or suffer
claims which were unknown or unsuspected at the time this Agreement was executed and which, if known by me at that
time, may have materially affected my decision to execute this Personal Release. I acknowledge and agree that by
reason of this Personal Release, I am assuming any risk of such unknown facts and such unknown and unsuspected
claims.
A GENERAL RELEASE DOES NOT EXTEND TO CLAIMS WHICH THE CREDITOR DOES NOT KNOW OR SUSPECT TO EXIST IN
HIS FAVOUR AT THE TIME OF EXECUTION THE RELEASE, WHICH IF KNOWN BY HIM MUST HAVE MATERIALLY
AFFECTED HIS SETTLEMENT WITH THE DEBTOR.
Notwithstanding such provisions, this Personal Release shall constitute a full release in accordance with its terms. I
knowingly and voluntarily waive that any statue, law or rule of similar effect.
I affirm/swear that neither I nor anyone acting for me gave or agreed to give anything of value to any member of the
production staff, anyone associated in any manner with the Producer, or any representative of the Producer. I
understand that the Producer’s policy prohibits such payments, and I further understand that the Producer’s policy
prohibits payments for arranging personal appearances and that failure to disclose to the Producer any such
arrangement constitutes a breach of this Agreement. I shall notify the Producer immediately if any person attempts to
induce me to violate the foregoing or to do anything dishonest.
I represent and warrant that I am at least 19 years of age and that I have the full, complete and unrestricted right and
authority to enter into this Agreement; or if I am under 19 years of age, I understand that my parent or legal guardian
must also sign this Personal Release where indicated below. I am not a candidate for public office and I agree that my
appearance in the Event, Langley Has Talent, if any, shall not be deemed to be a performance and is not employment
under any collective bargaining agreement or performing arts guild and does not entitle me to wages, salary, residuals,
corporate benefits or other compensation under any such collective bargaining agreement or otherwise.
I agree not to disclose or discuss events related to the Event or authorize any third-party to do the same, without the
prior approval of the Producer’s authorized representative. I shall not directly or indirectly issue or permit the issuance
of any publicity whatsoever with respect to the Event or my appearance in connection therewith. I shall not use the
name of the Event in anyway at anytime without the Producer’s prior written approval in each instance.
Notwithstanding anything contained herein, I hereby acknowledge that the Producer shall not prevent me from
performing, discussing, and/or publicizing my act separate (but without mentioning or referring in any way to the Event,
Langley Has Talent or my involvement with the Event).
I hereby authorize the Producer and any person or entity designated by the Producer to investigate, access, and collect
information about me and about any of the statements made by me in my application, this Agreement, or any other
written or oral statements I make in connection with the audition process and/or my participation in the Event. I further
authorize the individuals conducting investigations or examinations of me to disclose to the Producer their licensees,
successors, assigns, and representatives all information in connection therewith, and authorize the Producer to utilize
such information in selecting participants for the Event. I also unconditionally and irrevocably release and forever
discharge the Producer, the persons or entities designated by the Producer, and all such parties and persons from any
liabilities arising out of or in connection with such investigation or examination.
I acknowledge that I have the right to seek independent legal advice regarding any and all legal contracts/forms prior to
entering any legal agreement. This includes any minor having his/her legal guardian seeking legal advice on their behalf,
prior to entering into any legal agreement.
IF ANY CONTROVERSY OR CLAIM ARISING OUT OF OR RELATING TO THIS AGREEMENT, OR THE BREACH OF ANY TERM
HEREOF, CANNOT BE SETTLED THROUGH DIRECT DISCUSSIONS, THE PARTIES AGREE TO ENDEAVOR FIRST TO SETTLE THE
CONTROVERSY OR CLAIM BY MEDIATION CONDUCTED IN THE CITY OF LANGLEY OR LANGLEY TOWNSHIP, BRITISH
COLUMBIA AND ADMINISTERED BY AN AGREED UPON PARTY. IF A CONTROVERSY OR CLAIM IS NOT OTHERWISE
RESOLVED THROUGH DIRECT DISCUSSIONS OR MEDIATION, IT SHALL BE RESOLVED BY ARBITRATION CONDUCTED IN
THE CITY OF LANGLEY OR LANGLEY TOWNSHIP, BRITISH COLUMBIA, AND ADMINISTERED BY AN AGREED UPON PARTY.
THE MEDIATOR OR ARBITRATOR SHALL BE EXPERIENCED AND LICENSED TO PRACTICE IN THE PROVINCE OF BRITISH
COLUMBIA. I ACKNOWLEDGE THAT IN NO EVENT WILL I BE ENTITLED TO SEEK INJUNCTIVE RELIEF OF ANY KIND IN
CONNECTION WITH THIS PERSONAL RELEASE OR OTHERWISE IN CONNECTION WITH MY PARTICIPATION IN THE
AUDITION PROCESS OR THE EVENT, “LANGLEY HAS TALENT,” OR AGAINST ANY OF THE RELEASED PARTIES.
As used herein, “the Producer” shall include the Rotary Club of Langley (doing business as – DBA “Langley Has Talent”) in
partnership with the Rotary Club of Langley Central, Rotary Club of Langley Sunrise, Rotary Club of Aldergrove, its
licensees, successors, and assigns, and each of their respective parents, subsidiaries, and affiliates, and each of their
respective officers, directors, members, representatives, successors, licensees and assigns. I agree that the Producer
may license, assign, and otherwise transfer this Agreement and all rights granted by me under this Agreement to any
person or entity.
This Agreement shall be interpreted under the laws of British Columbia without regard to the conflicts of law provisions
thereof. This is the complete and binding agreement between the Producer and me, and it supersedes all prior
understandings and/or communications, both oral and written, with respect to its subject matter. The illegality,
invalidity or unenforceability of any provision of this Agreement shall in no way affect the validity or enforceability of
any of the remainder of this Agreement, which shall be enforced to the maximum extent permitted by law. This
Agreement cannot be terminated, rescinded or amended, except by a written agreement signed by both the Producer
and me. The Producer reserves the right to remove me from the Event at any stage if I fail to comply with the terms and
conditions of this Release.
I acknowledge that I have the right to seek independent legal advice regarding any and all legal contracts/forms prior to
entering any legal agreement. Legal advice can be obtained at no cost to the Producers prior to entering any legal
agreement. This includes any minor having his/her legal guardian seeking legal advice on their behalf.
Please read this agreement carefully. You are giving up certain legal rights under this agreement, including, without
limitation, your right to file a lawsuit in court with respect to any claim arising in connection with this agreement.

have caused said individual to execute said agreement, that I will not instruct, authorize or permit said individual to
disaffirm the foregoing agreement, and that I will indemnify the Producer against all claims, liabilities and expenses with
respect to said agreement, and that, knowing of the Producers reliance hereon, I agree to cause said person to adhere to
all the provisions of said agreement. That, I am entitled to seek independent legal advice, at no cost to the Producers,
prior to entering any legal agreement.
