CHRISTMAS ASSISTANCE
REGISTRATION
Will you need assistance this year for Christmas toys for your children 12 years old
and younger? Please read carefully.
To Receive Toy Assistance, You Must Apply in Person:
Monday, October 12th through Saturday, October 17th
October 12-16 - 9:00 AM - 6:00 PM
October 17 - 9:00 AM - 1:00 PM

All applicants must bring the following required documents to qualify:
1.
A valid for of photo ID for applicant (i.e., US Drivers License, US Passport, State ID, School ID,
Green Card, etc.)
2.
Proof of residence (utility bill, bank statement, etc.). Must be within the last month
before registering.
3.
Proof of ALL income, earned and unearned (check stubs, unemployment, SSI, Pensions, Veterans
Benefits, Disability, Food Stamps, Child Support, AFDC, WIC, Family Assistance must be accompanied by a letter from the relative providing the assistance stating how much, etc.) - documents
MUST have a dollar amount on them, we cannot accept a Food Stamp card or Appt. Letter Food
Stamps cannot be your only form of income.
4.
Proof of rent, mortgage, or subsidized housing (Receipt, Lease, or Letter from Landlord)
5.
Proof of ALL other expenses (car loan, medical bills, credit cards, other loans, etc.)
6.
All current utility statements (electric, water, gas, cable, phone, etc.)
7.
Birth Certificates for all children to be registered.
8.
Guardians/Grandparents need custody papers or note from the parents along with the Parents ID
and all information listed above.
9.
Please have a list of all clothing/shoe sizes for children with you.
10.
Applications will NOT be processed without ALL of the above information. DO NOT
apply for assistance if you have or plan to apply with another agency.

Muy importante, los siguientes documentos son necesarios para solicitar y calificar:
1.
Una forma valida de identificacion con foto de el aplicante (es decir, Licencia de conducir de
EE.UU., Pasaporte de los EE.UU., identificacion del estado, identificacion escolar, Green Card, etc.)
2.
Comprobante de domicilio (facture de servicios publicos, estado de cuenta bancaria, etc). Debe
ser del mes previo a inscribirse.
3.
Prueba de TODOS los ingresos, ganados y no ganados (talons de cheques, desempleo, SSI,
pensiones, beneficios de Veteranos, discapacidad, cupones de alimentos, manutencion de los
hijos, AFDC, WIC, asistencia familiar debe ir acompañada de un carta del familiar que provee la
asistencia estableciendo cuanto, etc.) - los documentos DEBEN tener una cantidad de dinero en
ellos, no podemos aceptar una tarjeta de cupones para alimentos o la carta de la cita. Cupones de
alimentos no pueden ser la unica forma de ingreso.
4.
Prueba de la renta, hipoteca, o viviendas de proteccion oficial (Recivo, arrendamiento, o carta del
propietario)
5.
Prueba de TODOS los demas gastos (prestamo de vehiculo, cuentas medicas, tarjetas de credito,
otros prestamos, etc.)
6.
La declaracion de TODOS los servicios publicos. (electricidad, agua, gas, cable, telefono, etc.)
7.
Acta de nacimiento de todos los niños a registrar.
8.
Tutores/Abuelos necesitan documentosde custodia o nota de los padres junto con
identificacion de los padres y toda la informacion que aparece arriba.
9.
Por favor, tener una lista de todos los tamaños de la ropa/calzado para niños con usted.
10.
Las solicitueds NO seran procesadas si no tienen TODA la informacion anterior. NO solicite ayuda
si aplico o va a aplicar con otra agencia.
