To Receive Christmas Gifts, Parent or Legal Guardian Will Need To Bring The
Following Documentation; To Receive A Christmas Food Basket, One Adult In
Household Will Need to Bring the Following Documentation:
1. VALID PHOTO ID (Driver’s License preferred), with correct address. If the address is not current, please bring a
utility bill in your name showing the current address. MUST be a resident of Belleair, Clearwater, Dunedin, Largo (zip
codes 33770, 33771, 33774, 33779), Oldsmar, Palm Harbor, Safety Harbor or Tarpon Springs.
2. SOCIAL SECURITY CARDS for the parent/legal guardian and all children age 12 and under.
3. BIRTH CERTIFICATES for all children age 12 and under (if applying for Christmas gifts).
4. Proof of Need: Must bring one of the following in applicant’s name:
- Section 8 or HUD lease documents
- Medicaid eligibility letter

- If you do not receive the above government assistance,

please bring proof of monthly expenses (rent, electricity, telephone,

insurance, etc.), and all income sources (most recent pay stub for each parent or
3
guardian to show monthly household income.
5. Please know clothing sizes and gift wishes for each child age 12 and under (if
applying for Christmas gifts).

Se puede tomar diariamente un número limitado de aplicaciones.
Para recibir regalos de Navidad, Padre o tutor legal tendrá que llevar la
siguiente documentación; para recibir una canasta de alimentos de Navidad, un
adulto en casa debe llevar la siguiente documentación:
1. DE IDENTIDAD CON FOTOGRAFÍA (licencia de conducir prefiere), con la dirección correcta. Si la dirección no
está actualizado, por favor traiga una factura de servicios públicos a su nombre que muestra la dirección actual. Debe ser
un residente de Belleair, Clearwater, Dunedin, Largo (33770, 33771, 33774, 33779), Oldsmar, Palm Harbor, Safety
Harbor or Tarpon Springs.
2. TARJETAS DE SEGURO SOCIAL para que el padre / tutor legal y todos los niños de 12 años de edad.
