This is the 3 rd international conference on prevention and management of chronic
conditions. This scientific meeting is designed to expand and exchange knowledge &
practice on prevention and management of chronic conditions in the national and
international level in term of improving outcome, quality and patient safety, role of family
and community, evidence based practice and success models, palliative care, and future
direction for prevention and management of chronic conditions. The audience will include a
wide range of national and international experts and participants who interested in the area
of chronic care.
High lights of this international conference compose of keynote and panel
discussions in global perspective in prevention and management of chronic conditions and
global responses to new science in quality and safety from the experts in these fields.
