2015 Nine Star Honor Band Participant Contract

2015 NINE STAR HONOR BAND PARTICIPANT CONTRACT
The Nine Star Honor Band is sponsored by the Florida Bandmasters Association (FBA).
The Honor Band is comprised of students that will have completed ninth grade at the end
of the 2014-2015 school year.
Directors, students, and parents/guardians should read the accompanying materials
carefully and sign to indicate agreement with the principles and rules stated.
Release for Media:
The signatures on this contract grant permission to the FBA to reproduce, print, and publish photographs and
audio/video recordings of the participating student if selected to the Nine Star Honor Band. This permission is
inclusive of all rehearsals, activities, and performances connected with the Nine Star Honor Band and the FBA
Summer Convention. I also acknowledge the participating student's name may be published in a concert
program and will appear in a list of Nine Star Honor Band students on the FBA website available to the FBA
members.
Registration and Chaperones:
•
All attendees (music directors, students, chaperones) must be registered to attend the FBA Summer
Convention.
•
Only directors may register students and chaperones. Only directors may pick up registration
materials at the convention site.
•
All participating students must be chaperoned. At least one chaperone, other than the music director, is
required from every school with selected students.
•
Directors must register for the FBA Summer Convention and be current members of the FBA in order
for students to participate.
•
Convention name badges must be worn at all times.
•
The director must appear in person at the registration desk and pay appropriate fees to replace "lost"
badges. The original registration receipt must be shown to obtain replacement badges.
•
Chaperones are required to check all rooms prior to leaving the hotel for any damages.
Student Rules:
•
Students accepted for participation in the Nine Star Honor Band agree to attend the FBA Summer
Convention and participate fully in all scheduled rehearsals and performances of the Nine Star Honor
Band. Dates for the FBA Summer Convention and Nine Star Honor Band are July 9 & 10, 2015. The
event will take place at the Hilton Daytona Beach Resort Oceanwalk Village (100 North Atlantic
Avenue, Daytona Beach, 32118).
•
Curfew for students is 11:00PM to 6:00AM. All students must be in their rooms and remain there
during the curfew hours stated.
•
Any student violating the curfew policy will be subject to dismissal from the Nine Star Honor Band and
may be sent home at the expense of the student's parent/guardian.
•
Students must maintain reasonable quiet behavior in hotel rooms, both day and night. Students are
prohibited from playing their instruments in their hotel rooms.
•
Students may not visit or entertain others of the opposite gender in their hotel rooms.
•
Room doors should be locked at all times and extreme care should be taken near hotel windows and
balconies. Nothing should be thrown from windows or balconies. Students are also not allowed on
hotel roofs.
•
Students are not to visit students/guests in other hotels without their chaperone's permission.
•
Students should be transported only in school approved transportation. Students are not to be in private
cars unless specifically approved by the school and are accompanied by a chaperone.
•
No alcoholic beverages may be in any participating student's possession at any time.
•
The use of illegal drugs is absolutely prohibited.
•
The use of tobacco products by participating students is prohibited.
•
Participating students may not engage in social dates during the convention.
•
Participating students should wear appropriate clothing, including shoes, at all times. The FBA requests
that school dress code be observed.
Failure to comply with these regulations may result in the dismissal of the student from the Nine Star
Honor Band. Additionally, the principal and music director of the school from which the student has
been dismissed, prior to submitting future applications, are required to provide policies to the FBA
Executive Director which address the manner in which they propose to minimize the likelihood of a
reoccurrence of such conduct in subsequent years.
