This directory provides a list of TexanPlus HMO’s network providers. To get detailed information
about your healthcare coverage, please see your Evidence of Coverage.
About Your Healthcare Provider Directory
.....................................................................................................................
This Provider Directory contains a listing of Primary Care Physicians, Specialists and other
healthcare professionals that are contracted with TexanPlus HMO and are considered network
providers. Primary Care Physicians and Specialists are grouped by geographical location and
practice referral patterns. These physician groups are referred to as a “Local Physician
Organization” or “LPO”. The LPO that your selected Primary Care Physician is associated with will
be your LPO.
Choosing A Primary Care Physician

You will have to choose one of our network providers who are listed in this directory to be your
Primary Care Physician (PCP). The term “PCP” will be used throughout this directory. Generally,
you must get your healthcare coverage from your PCP. The best way to meet your healthcare
needs is to have your care coordinated by your PCP who knows you and your health history.
Changing Your Primary Care Physician

It is important for you to feel comfortable with your PCP. You may change PCPs at any time by
calling Member Services. After we receive your request, the change will be effective the first day of
the following month.
Getting Care from a Specialist
.....................................................................................................................
Talk to your PCP if you think you need care from a specialist. Your PCP will — help you determine
if you need to see a specialist, select one that meets your needs, and provide a referral. If you
would like to see a specialist of your choice and your PCP denies a referral, you have the right to
appeal. You will be referred to a specialist that is in the same LPO as your PCP. If you need care in a
specialty area that is not included in your LPO, your PCP will work with us to identify an
appropriate participating specialist.

Mental Health and Substance Abuse Services Through Optum

A mental health and substance abuse benefit provides coverage for addressing treatment of
depression and drug or alcohol abuse. We use Optum’s network of mental health and substance
abuse professionals to provide you a wide array of treatment options from acute inpatient care to
individual outpatient counseling. You will enjoy the sense of security that comes from knowing
that specialized care is available when you need it. Optum’s network providers and facilities are
listed in this directory, but we advise you to contact Optum directly for assistance in locating a
provider and seeking service.

What Happens When I Call Optum?

You will be connected to a supportive Intake Coordinator who will help answer questions you may
have related to your mental health and substance abuse concerns. Individual cases are handled
confidentially by a Care Manager.
Care Managers help people identify the nature of their problems and find resources to address
them. Highly-skilled care management staff, supported by a team of psychiatrists, possess the
clinical experience to meet your needs.
What Can Optum Do For Me?

An Optum staff member is available to you 24 hours a day, 7 days a week to:
Help guide you during mental health and substance abuse emergencies (in potentially lifethreatening emergencies, you should always call 911 first, if available in your area).
Answer questions about your mental health and substance abuse benefits.
Provide names of contracted counselors and physicians in your area.
Monitor your progress during treatment and help you receive the best services possible.
Help you resolve problems or concerns you might have regarding the treatment you receive.
Coverage for services is based upon your eligibility and benefit plan limitations at the time services
are provided. Please refer to your Evidence of Coverage for a confirmation of current mental
health and substance abuse benefits.

Network Providers

The “network providers” listed in this directory have agreed to provide you with your healthcare,
vision, and dental coverage. You may go to any of our network providers listed in this directory;
however, some services may require a referral. If you have been going to one network provider,
you are not required to continue going to that same provider. In some cases, you may get covered
services from non-network providers.
Pre-Service Authorization

Certain healthcare services, such as specialist services, hospitalization, and outpatient surgery,
may require a prior authorization in order to be covered. If you do not obtain prior authorization,
you may be financially responsible for the services. Your PCP should process referrals for these
services on your behalf.
Claims
.....................................................................................................................
If a non-contracted physician or healthcare provider sends a bill directly to you for any healthcare
services, do not pay the bill. Any bill(s) sent directly to you must be submitted to us as soon as
possible for processing and determination if you are liable. The claims address is on the back of
your ID card.
Emergency Room Services
.....................................................................................................................
You must use plan providers except in emergency or urgent care situations or for out-of-area renal
dialysis or other services. If you obtain routine care from out-of-network providers neither
Medicare nor TexanPlus HMO will be responsible for the costs.
You are always covered for medical emergencies. Emergency health services are provided by
qualified physicians and healthcare providers and are needed to evaluate or stabilize an
emergency medical condition. An emergency medical condition generally refers to an injury or
illness that happens suddenly and includes severe pain. Emergency medical conditions appear to a
prudent layperson that, without immediate medical attention, the illness or injury could result in:
Serious jeopardy to your health
Serious impairment to a bodily function
Serious dysfunction of any organ or part

Examples of emergency medical conditions may include:
Loss of consciousness
Intense and uncontrollable pain
Severe shortness of breath that prevents you from talking
An accident involving a head injury or possible broken bones
If you experience an emergency medical condition, do not hesitate to seek medical care. You are
not required to use contracted hospitals or doctors for emergency medical conditions.
Simply:
1. Call 911 or go to the nearest emergency medical facility.
2. Present your member ID card.
3. Notify your PCP within 48 hours or as soon as possible (or have someone do it for you).
4. Contact your PCP for any necessary follow up care or instructions. Continued poststabilization care by a non-contracted physician or healthcare provider will need a referral
from your PCP, or you may be asked to return to the service area for further follow-up care, in
order to receive in-network benefits.
Emergency services are eligible for coverage under your Evidence of Coverage and will be covered
regardless of whether the hospital is in our network. If you receive services in a hospital
emergency room, you are responsible for the copayment as specified in your Evidence of Coverage.
If you have an emergency admission to a non-contracted hospital, we may work with your PCP to
move you to a contracted hospital for continued post-stabilization care when it is safe to do so.

Acerca del Directorio de proveedores de atención médica

Este Directorio de proveedores contiene una lista de médicos de atención primaria, especialistas y
otros profesionales de atención médica que participan en TexanPlus HMO y que serán
considerados como proveedores de la red. Los médicos de atención primaria y los especialistas se
agrupan por su ubicación geográfica y por sus patrones de derivación a consultorios. Estos grupos
médicos son conocidos como una “Organización Local de Médicos” (Local Physician Organization,
LPO). La LPO a la que está asociado el médico de atención primaria que ha elegido será su LPO.
Cómo escoger un médico de atención primaria

Usted tendrá que elegir a uno de nuestros proveedores de la red que aparecen en este directorio
para que sea su médico de atención primaria (Primary Care Physician PCP). El término “PCP” se
utilizará en todo este directorio. Por lo general, debe obtener su cobertura de atención médica de
su PCP. La mejor forma de satisfacer sus necesidades de atención médica es coordinar su atención
con un PCP que lo conozca y conozca su historial médico.

sumamente calificado cuenta con el apoyo de un equipo de psiquiatras y posee la experiencia
clínica necesaria para satisfacer sus necesidades.

Un miembro del equipo de Optum está a su disposición las 24 horas del día, los 7 días de la
semana, para brindarle los siguientes servicios:
Guiarlo durante las emergencias de salud mental y abuso de sustancias (en caso de una
emergencia que implique posible riesgo de vida, siempre debe llamar primero al 911, si está
disponible en su área).

Responder preguntas sobre sus beneficios de salud mental y abuso de sustancias.
Brindarle los nombres de los asesores y médicos contratados en su área.
Monitorear el progreso de su tratamiento y ayudarlo a recibir los mejores servicios posibles.
Ayudarlo a resolver los problemas o las inquietudes que pueda tener respecto del tratamiento
que recibe.
La cobertura de los servicios se basa en su elegibilidad y las limitaciones del plan de beneficios al

Proveedores de la red

Los “proveedores de la red” que aparecen en este directorio han accedido a ofrecerle su cobertura
de atención médica, de la visión y dentales. Puede acudir a cualquiera de nuestros proveedores de
la red que aparecen en este directorio; sin embargo, algunos servicios pueden requerir una
derivación. Si ha estado yendo a un proveedor de la red, no se le exige que continúe con el mismo
proveedor. En algunos casos, puede recibir servicios cubiertos de proveedores que no pertenecen
a la red.
Autorización previa al servicio

Ciertos servicios de atención médica, como los servicios de los especialistas, las hospitalizaciones y
las cirugías ambulatorias, pueden requerir autorización previa para recibir cobertura. Si no
obtiene esta autorización previa, es posible que usted deba pagar por estos servicios. Su PCP debe

Reclamos

Si un médico o proveedor de atención médica no contratado le envía una factura directamente por
cualquier servicio de atención médica, no pague la factura. Envíenos toda factura que le envíen a
usted directamente lo antes posible para procesarla y determinar si usted debe pagarla. La

Servicios de la sala de emergencias

Usted debe usar los proveedores del plan a excepción de situaciones de emergencia o urgentes o
para diálisis renal u otros servicios fuera del área. Si obtiene atención de rutina de proveedores
fuera de la red, ni Medicare ni TexanPlus HMO serán responsables de los costos.

Usted siempre tiene cobertura para emergencias médicas. Los servicios de salud de emergencia
son proporcionados por médicos y proveedores de atención médica calificados y son necesarios
para evaluar o estabilizar una condición médica de emergencia. Una condición médica de
emergencia por lo general se refiere a una lesión o enfermedad que ocurre repentinamente e
incluye fuertes dolores. Las condiciones médicas de emergencia se presentan en personas no
expertas prudentes y, sin atención médica inmediata, su enfermedad o lesión puede tener las
siguientes consecuencias:
Grave peligro para su salud

This Provider Directory contains a listing of Primary Care and
Specialty Care Physicians that participate in TexanPlus HMO for
member access to medical care. These Primary Care and
Specialty Care Physicians are further grouped together by both
geographical location and practice referral patterns. These
physician groups are individually referred to as a “Local
Physician Organization” or “LPO” in the Provider Directory. The
LPO covered by this portion of the directory is called the Golden
Triangle Physician Alliance. If you select, or have already
selected, one of the Golden Triangle Physician Alliance Primary
Care Physicians, then any additional care you need that cannot
be provided by that Primary Care Physician, will be provided by
a referral to a specialist listed in this directory and within the
Golden Triangle Physician Alliance.
