






18 years of age
Minimum of 5 years with Lawful Permanent Residency and living in the United
States (3 years if married to a U.S. Citizen)
Residence and Social Security Card
Passports with the last 5 years of travel
Males 18-26 years of age: Bring Selective Service Registration Card of $45
money order for sworn statement
2 passport style photos
You are responsible for the USCIS
$680.00 filing fee (Note: you do not
have to bring the fee to our office)
Necessary Information





Addresses with dates of employment and
residence for the last 5 years (SSI recipients must bring letter)
Children: Name, Date of Birth, Address,
Residence Card number (if applicable)
Spouse: Name, Date of Birth, Address,
Social Security, Residence Card number
or Citizenship Certificate information
Dates of all Marriages and Divorces for
you and your spouse(s)
If you’ve been arrested or detained, you
need a copy of the Disposition-Must get
disposition at the courthouse where convicted
